- This is a Sources Sought Notice (Request for Information). The Government intends to establish a Blanket Purchase Agreement (BPA) for Glidewire, Glidecath, Bead Block, Pinnacle Sheaths and Destination Sheaths for the Department of Radiology at Tripler Army Medical Center, Hawaii. See attached item listing. Calls will be placed against the BPA as supplies are required. NAICS code is 339112. Interested parties shall reply to Pacific Regional Contracting Office via email to Annette.Pickering@us.army.mil. TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide the exact brand items and also provide a current copy of your price list for the items NO LATER THAN 22 OCTOBER 2012, 12:00 PM Hawaii Standard Time (HST) or 6:00 PM EST. Contractor MUST BE ABLE TO PROVIDE 100% of the item(s). This is a NO SUBSTITUTE procurement.
