- The proposed contract action is for services, which the Naval Medical Center San Diego intends to solicit this requirement on a sole source basis to Hi-Desert Memorial Health Care District. Under the authority of FAR 13.106-1(b), sole source justification is based on Hi-Desert Memorial Health Care District being the ONLY Emergent laboratory company that can provide these services within a 30 minute drive to Twentynine Palms Hospital. The laboratory at this facility can perform the necessary testing. There are no other reference laboratories within Naval Hospital Twentynine Palms vicinity that can perform patient sample testing expeditiously, as all other laboratories are more than 2 hours away. This is a combined solicitation/synopsis for Service, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-13-T-0010 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 621511. POP: 01 October 2012 to 30 September 2013 with 4 OPTION YEARS. All interested bidders shall submit quotations electronically by email to joseph.bancod@med.navy.mil or by facsimile at 619-532-5596, attention Joseph Bancod. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations are due on or before 18 October 2012 at 08:00 am Pacific Standard Time to be considered responsive. CLIN 0001: Pathology Frozen Section Interpretation. Service to include: Initial and final interpretation of surgical pathology cases provided under the following circumstances: 1.Individual cases needed frozen section interpretation at the request of the attending NHTP physician during period when a San Diego pathologist is not available to the frozen sections. 2.Fee is global and includes 50% professional component (to be reimbursed by HDMC to pathologist directly) and 50% to technical component. QUANTITY 01 UNIT LOT $____________________________________ POP 01 Oct 2012-30 Sept 2013. SITE OF SERVICE: NAVAL HOSPITAL TWENTYNINE PALMS CLIN 0002: Lab Services for Urgent Reference. The testing is to include any and all clinic pathology that cannot be performed within the Naval Hospital Laboratory Department. These will not be performed because of loss of capabilities (equip. malfunction, expiration or lack of proper reagents, or lack of personnel to run the test). Examples of such test are listed below. Naval hospital Twentynine Palms will deliver the items to be tested to the contractor facilities laboratory. The results will be called into the contractor facilities laboratory. The results will be called into the Naval Hospital with a hard copy faxed to (760)830-2159. The test to be provided will include, but are not limited to: CPT Code Test, 82375 Carboxyhemoglobin, 80202 Vancomycin Level, 83874 Mygoglobin, 85384 Fibrinogen, 83930 Osmo.Serum, 82140 Ammonia, 80170 Gentamycin Level, 838880 B-Type Quantitiatve Peptide(BNP), 85379 D-dimer. Quantitative, 86140 C-Reactive Protein(CRP). QUANTITY 511 UNIT EACH $__________________________________________________. POP 01 Oct 2012-30 Sept 2013.
