- Appendix C - DOL labor rates Appendix B - Subcontracting Plan Appendix A - Price Matrix 2012NSD06MAINT01 Solicitation 2012NSD06MAINT01 - GSA HEARTLAND REGION MAINTENANCE SERVICES This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, under the authority of FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 2012NSD06MAINT01 is being issued as a Request for Proposals. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-60. The associated North American Industrial Classification System (NAICS) code for this procurement is 811213 with a size standard of $10 Million. This requirement is a full and open competition requirement and all qualified contractors may submit proposals. The requirement is to provide telecommunications maintenance services, telecommunications equipment, customer premise equipment, telephone instruments and associated services such as cabling, data racks, patch cords and labor for repair or programming/reprogramming of equipment to GSA Heartland Region 6 customers within the boundaries of Missouri, Kansas, Iowa and Nebraska. The contract will be awarded for a period of performance not-to-exceed five years; one year base period and four one-year option periods. The provision at 52.212-1, Instructions to Offerors-Commercial Items, and included addendum, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The factors to be used in proposal evaluation are Technical, Past Performance and Price. Contractors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with the proposal. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, including the clauses marked within this clause, applies to this acquisition. Proposals are due on or before November 2, 2012 at 3:00pm CDT. All proposals must be submitted via email to the Contracting Officer at shellie.heislen@gsa.gov by the established date and time. Any questions regarding the solicitation are to be addressed to the Contracting Officer at shellie.heislen@gsa.gov no later than October 19, 2012.
