- STATEMENT OF WORK ELEVATOR INSPECTION SERVICES To assure that the Elevators, Dumbwaiters and Cartlifts at the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center are operating properly and safely the Department of Veterans Affairs require no less than two (2) semi-annual inspections per twelve calendar month period be performed on each of its Elevators, Dumbwaiters and Cartlifts in accordance with applicable National Fire Protection Association (NFPA) and American Society of Mechanical Engineers (ASME) Standard A17.1. These specifications cover the Semi-Annual Maintenance Inspection of Elevators, Dumbwaiters and Cartlifts and Annual Task of Governor and Safety Devices on said elevators, dumbwaiters and Cartlifts during the months specified. The American Standard practice for the inspection of elevators, inspector's manual, a17.s-1979, or latest edition and the American National Standard Safety Code for elevators, dumbwaiters, escalators and moving walks - ANSI A17.11978 or latest edition, will be used by the inspector as a guide in inspections. The inspections shall be thorough and cover all items as specified, as well as any other parts and assemblies not listed thereunder, in order to make a proper inspection. Parts shall be removed, where necessary, determine the exact condition of parts which could not otherwise be determined, and the equipment shall be cleaned (where necessary) to make inspection. A minor and a major inspection will be completed by a qualified licensed elevator inspector. The minor inspection shall include all phases of elevator operation, functioning of all safeties, operating controls, lighting, exhaust systems, oil levels (as applicable), cables, stops, hoist ways, shafts and elevator pits and mechanical rooms The major inspection will consist of all above items plus act as a witness for the annual safety testing of all units. Upon completion of inspections and within then (10) business days after each inspection, a written report of inspection will be provided to the Chief of Engineering Service notifying the Department of Veterans Affairs of any discrepancies. Any unsafe conditions encountered during the inspections will be reported immediately to the Contracting Officer's Representative (COR) or designee. A written certification for each Elevator, Dumbwaiter and Cartlift will be provided. ANNUAL TEST: Safeties shall be subject to an annual running test. Note: The Contractor will provide Inspection Services ONLY and will not perform any maintenance. SCHEDULE: The Department of Veterans Affairs requires the semi-annual inspections to be completed six (6) months apart. The first semi-annual inspection will be made during the month of December of each respective year; the second semi-annual inspection will be made during the month of June of each respective year. Pressure tanks shall, after being thoroughly cleaned as required by rule 1002.5b-1 of American National Standard Safety Code for Elevators, Dumbwaiters, Escalators and Moving Walks, ANSI A17.11-1977, be internally inspected and shall, after such inspection, be subjected to a hydrostatic test with 150% pressure applied for one minute. Piston rods of roped hydraulic elevators, dumbwaiters, and Cartlifts shall, after being cleaned as required by Rule 1002.5b-2, be exposed and inspected and where pitted or worn to a diameter less than the root diameter of the threads shall be replaced. Inspect flexible hose and fitting assemblies and flexible couplings as require by rule 100.4-d, except under pressure of three times the working pressure for hose and fitting assemblies. REPORTS: The Contractor shall provide a complete report upon completion of the inspection. The contractor shall clearly indicate any adjustments, repair, or replacements necessary to correct trouble encountered; shall definitely justify such recommendations, and shall furnish specifications sufficient to enable the Department of Veterans Affairs Medical Center to purchase the material required for such purposes. The Contractor shall advise whether the elevators' parts have been kept clean and lubricated where necessary. A complete, signed report should be provided to the administrative assistant, facility management service, room i-101. GENERAL: The Contractor shall furnish all labor, equipment, supplies, parts, supervision, management and transportation necessary to perform Elevator Inspection Services in accordance with (IAW) the specifications, terms and conditions of this contract for the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center for a period of one (1) year to commence October 1, 2012 through September 30, 2013, with four (4) option years for renewal. All work is to be performed in accordance with (IAW) Federal, State and local laws and ordinances by a State of Mississippi licensed inspector with five (5) of more years of experience. IDENTIFICATION: All Inspectors must have visible on their uniform, their name and company's logo. When there are more than one Contractor reporting, one shall be designated as a "supervisor" and be fully certified in Elevator inspection. Non-certified employees may assist the supervisor in the performance of the elevator service as long as the non-certified employee is with and visible to the supervisor. The supervisor shall assume all responsibility for the non-certified employee. CERTIFICATION OF CONTRACTORS: Contractors shall be certified to perform Elevator Inspection Service. The Contractor shall furnish a copy of up-to-date and current State certification (showing categories) to the Department of Purchasing and Contracting and to the COTR upon request and prior to the start of the contract. CONFORMITY TO REGULATIONS: The Contractor shall conform to Federal, State and local regulations governing, Elevator Inspection. INTERFERENCE TO NORMAL FUNCTIONS: The Contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facilities, fire protection systems and pass of facility patients personnel, equipment and carts. HOURS OF OPERATION: The normal hours of operation for all facilities are 7:00 am to 5:30 pm, Monday through Friday. SCHEDULE OF VISITS: The Contractor shall provide a schedule of when Inspection Services shall be scheduled to the Contracting Officer Representative (COR) within fifteen (15) calendar days prior to the anticipated inspection date. Inspection tasks shall be scheduled so as to be performed on definite appointed days and in no event shall the Contractor perform inspections outside regular agreed upon work hours without prior approval from the COR. In the event climate or weather conditions make it unsuitable for inspection services, the entire service date will be re-scheduled to a date and time satisfactory to the Contractor and the COR. Postponement will not reflect any penalties assessed to the Contractor or to the Government. POINT OF CONTACT (POC): The Contracting Officer shall delegate the COR to monitor the Contractor's performance. REQUIRED CONTRACTOR REPORTING: The Contractor will sign in and out when performing Inspection services. The COR shall provide instructions as to the POCs and locations of sign-in/out. All Inspection Services shall be documented. The COR shall brief the Contractor on the preparation of this report. The Contractor is responsible for supplying, completing and submitting reports as required and requested by Federal, State or local ordinances which pertain to any duties contained in the contract.
