- Request for Bid Proposal SUBJECT: One Time Bid Proposal 16 October 2012 BID NUMBER: FPFY13002 Item 1: Used Lead Acid Batteries AMOUNT BID Item1 ________________ Item 1: One Lot of Used Lead Acid Batteries. The estimated weight of the Lot is 46,000 lbs. The lot will not be split up. The bid offered will be for the entire lot. Batteries are on pallets and will be prepared IAW DOT specifications. Interested bidders are encouraged to visit the storage location to obtain a better understanding of the product composition. The property can be viewed 16-18 October 2012 between the hours of 0800- 15:00. Bidder must call ahead a minimum of 24hours for inspection coordination. The point of contact for viewing is Mr. Joe Hoer at (337) 531-7556. Bids will not be discussed verbally on site. All bids will be delivered on a sealed basis via FEDBIZOPS, facsimile (ATTN: Tim Fitzgerald) or email transmission. In the event of a "tie bid", the earliest bid will have priority. The winning bidder will be notified via email transmission. LOCATION OF RECYCLABLE MATERIAL: Located on South Fort Polk Bldg 3620. Seller will have forklift available on site. There are no loading ramps available. Loading will occur on Flat Bed Trailer only supplied by the winning bidder. Fort Polk is located approximately 80 miles north of Lake Charles, Louisiana, 60 miles west of Alexandria, Louisiana and 120 miles south of Shreveport, Louisiana. Hours of operation: 0800-1600, Monday through Friday, except for Federal Holidays and Training Holidays (call ahead for schedule). Loading and weighing of vehicles must be completed prior to 1530 and will be done on an official government scale. Please examine the following specifications: ARTICLE I: 1. The DPW / ENV / Qualified Recycling Program (QRP) agrees to sell to the Purchaser, as outlined in Article I, Item 1. Item 1: Used Lead Acid Batteries with an estimated weight of 46,000 lbs (see above for detailed description) ARTICLE II: The Purchaser. 1. The Purchaser agrees to schedule the pickup of Item 1, no later than the close of business on 08 November 2012. Purchaser will notify the seller at a minimum of 48 hrs prior to scheduled pick up in order to eliminate any waiting time at weigh-in. The QRP point of contact for this sale is Mr. Joe Hoer at (337) 531-7556. 2. The Purchaser will insure that drivers are aware that vehicles used in transporting awarded items are weighed-in prior to pick-up and are weighed again, once loaded and prior to leaving the installation. 3. The Purchaser agrees to pay DPW-ENV-QRP, within 15 days of pickup. a. PAYMENT sent to: DPW / ENV / Qualified Recycling Program (QRP) ATTN: Tim Fitzgerald 1647 23rd St., Bldg. 2516 Fort Polk, Louisiana 71459-5339 b. Payable to: Fort Polk Qualified Recycling Program (QRP) c. Payments made in negotiable instruments, money orders and checks. Checks accepted with a guarantee of credit from issuing bank. 4. The sale price is determined by the closing bid, in the event that there is a matching bid, the first bid will have priority. This will be determined by the date and time of bid submittal via FEDBIZOPS, email or facsimile transmission. 5. Securing the load is the sole responsibility of the Purchaser and or his agent. ARTICLE III: General Provisions 1. Default: If the Purchaser fails to pay for and or remove the awarded within the time allowed by this agreement, DPW-ENV-QRP will send the Purchaser a default notice that must be cured within 72 hours of notification. If Purchaser fails to cure default within the stated period, the Purchaser shall lose all rights, title and interest, which he may otherwise have acquired in and to such property as to which a default has occurred. 2. Weight Certification: Empty trailer or vehicle will be weighed on government scales located at Fort Polk, Louisiana. The loaded trailer or vehicle will be weighed at same location prior to departing Fort Polk, Louisiana. Both, the Purchaser or representative and DPW-ENV-QRP representative will be present and will certify the weight of the vehicle and vehicle load. Both parties will sign the weight ticket. All weight discrepancies will be resolved prior to departing Fort Polk. Certified weight ticket can be provided to Purchaser upon request. 3. Insurance: The purchase/ representative shall be required to provide and maintain during the entire period of this contract, at least the motor vehicle liability and bodily injury insurance as required by State and local laws. 4. Recycle Property. The purchaser or representative shall arrange with the POC for use of the facility and any government furnished equipment available for services. Purchaser or representative shall not make changes to Fort Polk facilities or equipment. Should the purchaser or representative desire to make changes the purchaser shall present the proposed change to the POC in writing. The POC will respond in a timely matter. 5. Confidentiality: Any and all information made available to the purchaser/representatives as a result of this contract, shall be kept confidential. Failure to comply with this provision is a basis for termination of the contract. 6. Transportation: The purchaser/representative must possess a valid driver's license, insurance and registration of vehicle for access Fort Polk. Installation can only be made through gates at LA 10 ( #7) and LA 469 (#4). 7. Indemnification: Purchaser/representative aggress to hold and save harmless at all times the United States (including Fort Polk QRP, its agents, representative, and employees) against any and all claims, demands, suits, judgments, costs, charges, and expenses arising out of or in connection with any loss, damage or injury resulting from the negligence or other fault of the purchaser, its agents, representatives, and employees. The purchaser will release the U S Government and Fort Polk QRP from any and all liability from loss or damage to property and merchandise used by the purchaser/representatives in the operation of this event due to theft, fire, storm, flood, and damage or destruction through any force of nature or otherwise. 8. Damages: The purchaser shall be financially responsible or liable for any damages to the premises/building resulting from acts of omission or errors of the purchaser and their employees/representatives. Any acts of vandalism or theft shall be reported immediately to the installation Provost Marshall and Fort Polk QRP.
