"Renovate Second Floor 1W for Ambulatory Care" This notice supercedes notice Z-Renovate 2nd Flr 1W for AMBULATORY CARE Project 581-11-103; POSTED Oct 5, 2012 @ 10:01 AM, REFERENCING VA249-13-B-0015; Note: The only change is the solicitation number; VA249-13-B-0040 in lieu of VA249-13-B-0015. I. General Intention A. Contractor shall furnish all labor, material, equipment, and supervision necessary to complete Project No. 581-11-103, "Renovate Second Floor 1W for Ambulatory Care" at the VA Medical Center, 1540 Spring Valley Drive, Huntington, WV 25704. B. Site visit dates and times will be published with the information for bid. C. Prior to commencing work, general contractor shall provide proof that an OSHA certified "competent person" (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. II. Magnitude of construction The magnitude of this requirement is between $2,000,000 and $5,000,000. III. Place of Performance The place of performance will be: VA Medical Center, 1540 Spring Valley Drive, Huntington, WV. IV. Set-Aside This is a service disabled veteran owned small business (SDVOSB) set-aside. The SDVOSB must be certified as a service disabled veteran owned small business (SDVOSB) with vetbiz, and must be registered in the system for Award management (SAM). V. Statement of Bid Item Project 581-11-103; "Renovate Second Floor 1W for Ambulatory Care" Statement of Bid Item: This project will entail demolition work and construction work. Demolition work includes removal of existing floors, walls, doors, sinks, ceilings and other associated items to prepare for new finishes. Construction work includes second floor renovation for an ambulatory care wing at the Huntington VA Medical Center including demolition and new work for approx. 11,000 sq. ft. The renovated area will include 28 exam rooms, 3 waiting areas with spaces for 7 teams' clerks, support spaces and offices. Renovation includes construction of new floors, walls, doors, frames, countertops, shelving, and other ambulatory related items.
