- This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this Sources Sought Announcement is for market research purposes and to gain knowledge of potential qualified small business sources interested and capable of performing the work. Responses to this notice are required to be in writing and shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (g) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov. The Department of Veterans Affairs Network Contracting Office 22 is seeking sources for establishing a contract for the services of a Board Certified Cardiovascular Perfusionist. Services are to be performed at the VA San Diego Healthcare System, located at 3350 La Jolla Village Drive, San Diego, CA 92161. Perfusionist must be available twenty-four (24) hours a day, seven (7) days a week, including all national holidays. Perfusionist must be on-site to set up heart lung machine within one (1) hour of notification of need for services. It is the VA's intention to solicit proposals on a competitive basis for an indefinite delivery, indefinite quantity, Firm-Fixed Price contract. Proposed prices shall be all inclusive of professional fees, malpractice insurance, benefits and contractor's fees. The contract will be awarded as a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. This solicitation will be issued as a commercial item in accordance with FAR Part 12 and FAR Part 15. Applicable NAICS Code is 561320. The solicitation is expected to be issued on or about November 19, 2012 with responses due on or about December 24, 2012 31, 2013.
