Description
Wage Determination Request for Quotes NAF Concessionaire Contract/Special & General Provisions Statement of Work This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4608-13-Q-S003, in accordance with FAR Parts 12 and 13, and is in accordance with provisions and clauses effective in Federal Acquisition Circular (FAC 2005-23 effective 26 Dec 2007). The North American Industry Classification System (NAICS) number is 812111, and the business size is $7.0 million. The proposed acquisition is set-aside for small businesses. Small businesses, woman-owned, and disabled veteran-owned businesses are encouraged to participate by submitting quotes. Description: This requirement is to operate a Barber Shop concession in accordance with the attached Statement of Work for a base period to begin on or about 1 April 2013 through 31 March 2014 with four (4) option years. The assigned space is located in the Stripes Enlisted Club, Bldg 1955 Rickenbacker Rd., Barksdale AFB, Louisiana and is 585 square feet or 32'6" x 18'. All barbers must possess a valid state barber's license. Attached Documents: * Statement of Work * NAF Concessionaire Contract/Special & General Provisions *RFQ *Wage Determination Evaluation Factors: ** Award will be made to the offeror who presents the lowest priced, technically acceptable offer. The following information shall be provided and will be evaluated to assess technical acceptability. Technical acceptability consists of a quotations containing evidence that the Contractor personnel is state licensed. One rating will be assigned to the technical acceptability factor: technically acceptable or not technically acceptable. A bid that does not include the items above will be deemed non-responsive. The technical acceptable offers will then be evaluated against the greatest percentage proposed, with award being made to the greatest percentage proposed of the technically acceptable offer. Offerors must use attached RFQ to provide proposed bids. "Notice to Offerors: This is a nonappropriated fund purchase and it does not obligate appropriated funds of the United States Government. Nonappropriated funds are generated by the military community through the sale of goods and services and the collection of fees and charges for participation in military community programs. This purchase does not involve federal tax dollars." Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this acquisition and the following addenda applies. Offerors shall submit their quote on letterhead stationery, provide unit prices with a grand total, and the contractor must remit a completed copy of the REPRESENTATIONS AND CERTIFICATIONS located at FAR 52.212-3, and the 252.212-7000 Offeror Representations and Certifications with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. Offers will be evaluated on the prices as offered. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be acceptable, and therefore will not be considered. Once an offer has been determined to be acceptable, and in compliance with all directions in this solicitation, the Government will award to the offeror with the best value to include price and proposed performance time. Quotes can be faxed to 318-456-2629 (in accordance with FAR provision 52.214-13 TELEGRAPHIC BIDS) or emailed to the attention of TSgt Jason Wilkin (jason.wilkin@us.af.mil) no later than 25 November 2012 at 2:00 p.m. (CST). Offers received after this date and time will be considered as late submissions in accordance with 52.212-1(f) and will not be evaluated or considered.
