Note: if a hardware upgrade is required for a new software version customer will receive a 10% discount off the list price for parts and labor. All new versions and maintenance releases as released. Operating computer Computer hardware not covered after 1 year warranty period. Customer responsible for any necessary hardware upgrades. Coverage included per below coverage details. Scheduling Priority scheduling over time and materials customers. Qualified on-site technicians within 5 business days of receipt of repair purchase order if remote trouble shooting ineffective. Qualified on-site technicians within 3 business days if remote trouble shooting ineffective. s New test/accessory purchase 10% discount 20% discount Coverage Details • Annual preventive maintenance and calibration: for each year this Software and Service Contract is in effect, we will provide a check-up of the Covered Products including Software updates, calibration, preventive maintenance and full functional testing of your NOTC System currently includes 51 Tests / Inspections: Electrical Console: 10 Tests/Inspections Chair Assembly: 13 Tests/Inspections System Enclosure: 11 Tests/Inspections/Replacements Computer System: 4 Tests/Inspections I-Portal® NOTC System: 13 Tests/Inspections • Premium Plan extended parts warranty (Premium Plan Only): Vendor will replace any parts needed for repair of covered products with a new, rebuilt or refurbished product of equal or similar features and functionality. The foregoing coverage applies only to failures arising under normal use and does not include: cosmetic damage; malfunctions or failures resulting from: spilled liquids, misuse, abuse, neglect, alteration, vandalism, problems with electrical power, acts of nature, unusual temperatures or humidity or damage caused by unauthorized repairs or modifications. • Vendor will make every attempt to service the equipment during normal business hours: Vendor will provide phone coverage from 8:00 AM to 4:30 PM EST and on-call coverage from 4:30 PM to 9:00 PM EST Monday through Friday. • Software updates and new version releases provided to contract customers as they become available. Releases that require field modifications will be installed in conjunction with PM or similar visits. Customers requiring a timelier update may schedule a separate trip at customer's expense. • Period of performance: coverage for initial contract will be 12 months from receipt of payment. In the event a renewal payment is late the contract may be backdated to the end date of the previous contract by mutual agreement, however, should an event requiring repair work, training or consulting happen prior to receipt of the contract renewal payment, services will be provided at the attached time and material rate and the new contract will begin on the date the renewal payment is received. • Lapse in coverage: Customer is responsible for problems encountered during non-covered periods. Should a customer wish to enter into Software and Service Contract after a lapse in coverage, any damage found during initial PM may incur additional charges. • Operating computer coverage: Computers are covered under the initial warranty and under premium plans, with the exception of storage capacity or unauthorized software, as per terms. Computer obsolescence is not within the scope of the contract. Hardware upgrades including additional hard drive space, if necessary, are the expense of the customer. Operating computer is provided solely for control of the supplied medical device. Operation has been verified and validated with Vendor loaded software only. Customer loaded programs may affect performance and problems that arise from loading additional software will not be covered.
