Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA2860-13-T-0001. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-61. This requirement is set aside 100% for small businesses. The North American Industry Classification System (NAICS) code is 621999 All Other Miscellaneous Ambulatory Health Care Services with a $10.0 million size standard. Award will be based on Lowest Price Technically Acceptable (LPTA). Written proposals are required (oral offers will not be accepted). Only one contract will be awarded. LINE ITEMS: 0001: Removal and Disposal of Medical Waste. Period of Perfomance: 01 December 2012 - 30 November 2013. Required place of performance is located at Malcolm Grow Medical Center, Building 1050 Joint Base Andrews NAF, MD 20762. The resulting contract will be a firm fixed price. Performance Work Statement (PWS) for Medical Waste Removal and Disposal 1.1 SCOPE OF WORK: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary for the transportation, incineration, sterilization, or other approved method of decontamination and final disposal of regulated medical waste generated by the 79th Medical Group (Malcolm Grow Medical Center), building 1050, Andrews AFB, Maryland. The contractor shall perform to the standards in this statement of work. The estimated quantities of work are 5,600 lbs per month or 67,200 lbs annually. 1.2 PERSONNEL 1.2.1 CONTRACT MANAGER: The contractor shall provide a contract manager who shall be responsible for the performance of work. The name of this person and an alternate(s) who acts for the contractor when the manager is absent shall be designated, in writing, to the contracting officer. 1.2.1.1 The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operations of this contract. 1.2.1.2 The contract manager and alternate(s) must be able to read, write, speak, and understand English. A complete working knowledge of federal, state, and local statutes regarding the transportation and disposal of regulated medical waste/medical waste is required. 1.2.2 EMPLOYESS: Contractor personnel shall be easily recognized by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company name and employee name. 1.2.2.1 Employee(s) shall wear protective clothing when handling regulated medical waste/medical waste (gloves, masks, etc.). 1.2.3 FEDERAL HOLIDAYS: The contractor is not required to provide service on the following Federal Holidays: New Year's Day, Martin Luther King Jr' s Birthday, Washington's Birthday, Memorial Day, Independence Day, and Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day. The contractor shall pick up all regulated medical waste/medical waste the next day following a federal holiday. SECTION C-2 - DEFINITIONS 2.1 STANDARD DEFINITIONS 2.1.1 Quality Assurance: The government will evaluate the contractor's performance using surveillance method. All surveillance observations shall be recorded by the government. When an observation indicates defective performance, the Quality Assurance Evaluator (QAE) will notify the contracting officer. The contracting officer reserves the right to inspect any and all facilities used by the contractor for the transportation, incineration/sterilization/disinfection and final disposal of regulated medical waste. 2.1.2 Quality Assurance Evaluator (QAE): A government person responsible for surveillance of contractor performance. 2.1.3 Quality Control: Those actions taken by a contractor to control the performance of services so that they meet the requirements of the statement of work. 2.2 TECHNICAL DEFINITIONS PECULIAR TO THIS SOW 2.2.1 SPECIAL MEDICAL WASTE: In Maryland, special (regulated) medical waste means a solid waste that is not excluded under Code of Maryland Regulations (COMAR) 26.13.11.03 and is composed of: a. "Anatomical material" is human or animal body parts, including tissues and organs. b. "Blood" is human or animal blood. c. "Blood-soiled articles" are any articles that contain blood in any form as a result of contact with blood. d. "Contaminated material" is: (1) Microbiological laboratory waste that contains an infectious agent and includes cultures and blocks of infectious agents and associated biologicals. (2) The feces of an individual diagnosed as having a disease that may be transmitted to another human being through the feces. (3) An article soiled with the feces of an individual diagnosed as having a disease that maybe transmitted to another human being through feces. (4) An article that has come into contact with a known infectious agent. e. "Sharps" means a syringe, needle, surgical instrument, or other article that is capable of cutting or puncturing human skin. 2.2.2 GENERATOR: For the purpose of Maryland's law for the Disposal of Special Medical Waste, COMAR 26.13.11.02 means the 79th Medical Group, Andrews Air Force Base, Maryland. 2.2.3 TRANSPORTER: For the purpose of Maryland's law for the Disposal of Special Medical Waste, COMAR 26.13.11.02, means the contractor. 2.2.4 INCINERATOR: A thermal device in which solid waste in incinerated/ cremated for the purpose of volume reduction. 2.2.5 FINAL DISPOSAL: The site or facility, utilized for ultimate disposal of solid waste by the sanitary landfill method. 2.2.6 SANITARY LANDFILL: A land disposal site employing and engineered method of disposing of solid wastes. SECTION C-3 - GOVERNMENT FURNISHED SERVICES 3. GENERAL: The government shall have the medical waste boxes properly packaged for shipment. 3.1 SERVICES: The government shall be responsible for weighing/ counting the medical waste boxes being shipped for disposal at Andrews AFB Maryland, building 1050. SECTION C-4 - CONTRACTOR FURNISHED ITEMS AND SERVICES 4.1 PERMITS: The contractor shall comply with all applicable county, state, and federal regulations pertaining to the transportation, incineration/ sterilization or other approved method of disinfection and disposal of regulated medical waste. The contractor shall provide the contracting officer with a copy of the following permits/licenses no later than 5 calendar days after bid opening: a. If the contractor does not own an incinerator/sterilizer/compactor, etc. to be used in this contract; a copy of the permit/license to burn or otherwise disinfect regulated medical waste. b. If the contractor does not own or operate an incinerator/sterilizer/compactor, etc., to be used in this contract; a copy of the agreement with the subcontractor to destroy or otherwise desensitize regulated medical waste, and a copy of the subcontractor's permit/licenses to burn or otherwise desensitize regulated medical waste. c. The contractor shall provide to the contracting officer proof of his authorization to dispose of incinerated or otherwise desensitized waste in an approved landfill. d. The contractor shall provide to the contracting officer a copy of all permits/licenses from state(s), federal/county(ies) for hauling regulated medical waste. e. The contractor will notify the contracting office in writing within ten work days of any permit, incinerator/compactor/sterilizer, etc., or landfill change requests that occur during this contract period. 4.2 PICKUP: From the 79th Medical Group, Bldg 1050, Andrews AFB, MD the contractor will make requested pickups Monday and Thursday of approximately 1,270lbs of regulated medical waste per week. 4.2.1 The contractor will provide the medical waste boxes, liner, tape, transportation, disposal, all documentation and any in-service requirements we may have. 4.3 METHOD OF DISPOSAL: The contractor shall transport the boxes of regulated medical waste from the government facility to an identified and permitted regulated medical waste incinerator/sterilizer/compactor. 4.4 CLEAN UP: The contractor shall clean any debris caused by their removal and loading of the regulated medical waste. The area shall be returned to the original condition prior to pick up as determined by the QAE. 4.5 CARTONS: The contractor shall provide a supply of cardboard boxes designed for the purpose of transporting regulated medical waste and properly labeled per county, state and federal regulations. An estimated 200 boxes shall be delivered each month to the 79th Medical Group (Malcolm Grow Medical Center), Bldg 1050 (loading Dock), Andrews AFB, MD. 4.6 BAGS: The contractor shall provide a supply of 3.0 milliliter thick, red hazardous waste bags properly labeled per county, state, and federal regulations. An estimated 300 bags shall be delivered each month to the 79th Medical Group (Malcolm Grow Medical Center), Bldg 1050 (loading dock), Andrews AFB, MD. 4.7 DELIVERY TICKETS: The contractor shall furnish a delivery ticket to the QAE at Bldg 1050, AAFB for each pick-up and delivery made. 4.8 INVOICES: The contractor shall provide a monthly invoice for the 79th Medical Group, AAFB MD for all services rendered for the month. a. A signed copy of each manifest will be provided with the monthly invoices. b. Weights and number of boxes will be entered on contractor provided documentation. c. The 79th Medical Group waste generator number will be annotated on each manifest. 4.9 INTERRUPTIONS IN SERVICE: The contractor shall notify the Contracting Officer immediately in the event of any interruptions in service and the appointed QAE. 4.10 TRANSPORTING WASTE: The contractor shall ensure that regulated medical waste is not transported with any hazardous waste as defined by the Resource Conservation and Recovery Act (RCRA). 4.11 The contractor shall provide copy of proof of each disposal made within the allotted timeframe. FAR 52.212-3 "Offeror Representations and Certifications - Commercial Items" applies to this acquisition and offeror(s) must include a completed copy with its offer. FAR 52.212-3 can be obtained from http://farsite.hill.af.mil or form http://orca.bpn.gov if registered in ORCA (Online Representation and Certifications application.) FAR CLAUSES: The clause at 52.212-4, Contract Terms and Conditions-Commercial Items is hereby incorporated.
