This is not a solicitation announcement. This is a sources sought notice for the identification of potential small business vendors with current schedule 65 II A - A 69 FSS contracts to provide Specialty Beds and accessories for Edward Hines VA Hospital, Fifth Avenue & Roosevelt Road, Hines, Illinois 60141; Jesse Brown VA Medical Center, 820 South Damen Avenue, Chicago, Illinois 60612; Lovell FHCC, 3001 Green Bay Road, North Chicago, IL; Zablocki VAMC, 5000 W National Ave, Milwaukee, Wisconsin; Wm S Middleton VA Hospital, 2500 Overlook Terrace, Madison, Wisconsin 53705; Tomah VAMC, 500 E Veterans St, Tomah, WI 54660; and the Iron Mountain VAMC, 325 E H Street, Iron Mountain, MI. Vendors must be current holders of schedule 65 II A - A 69 FSS contract. Multiple BPAs will be awarded with a performance period from approximately November 15, 2012 through November 14, 2017 with quarterly reports required. The purpose of this notice is to gain knowledge of potential qualified sources with current schedule 65 II A - A 69 FSS contract. Consistent with the VA's Veterans First Contracting Program, VA is particularly interested in identifying Service-Disabled Veteran-Owned Small Businesses and/or Veteran-Owned Small Businesses with the capability to perform. All interested and capable respondents are requested to provide the information below. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com), FSS contract number, identify which items vendor would be able to provide from current FSS contract that meet the item specifications, ability to delivery/service/pick-up 24/7 within 12 hours of notification, and ability to provide all service/maintenance of items 24/7 within 12 hours. Vendor is not required to provide all items but must indicate which items vendor can provide from current FSS contract. Responses to this notice should be sent via e-mail email to rose.birkholz@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 2:00 PM ET, Tuesday October 30, 2012. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM). The website for registration is https://www.sam.gov/portal/public/SAM/. Service-Disabled Veteran-Owned Small Businesses and Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov.
