Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ VA248-13-Q-0189. Capabilities statement and past performance must accompany submission. All submissions must be received no later than November 6, 2012 9:00am (EST). Pursuant to 38 USC 8127(d) and in accordance with Public Law 109-461, the Veterans Benefits, Health Care and Information Technology Act of 2006, this procurement is a competitive total set-aside for Small Business only. The North American Industry Classification Code (NAICS) for this procurement is 811219 with a small business size standard of $19 million. NOTE: The Government reserves the right to award without discussions, vendors shall submit the most favorable terms. Small businesses are encouraged to contract their nearest Procurement Technical Assistance Center (PTAC) if assistance is needed in preparing their quote. The offeror is responsible to monitor and download any amendments pertaining to this solicitation from the Federal Business Opportunities (FBO) website: http://www.fedbizopps.gov. In accordance with FAR 52.204-7, Required Central Contractor Registration prospective contractors must be registered in Central Contractor Registration (CCR). Firms may obtain CCR information at https://www.sam.gov/portal/public/SAM/ The provision at 52.212-1, Instructions to Offerors - Commercial items, applies to this acquisition. Offerors shall provide a firm fixed-price offer. The Government will award a contract resulting from this solicitation to the responsible offer whose offer conforming to the solicitation will be most advantageous and Best Value to the Government. Successful contractor must meet all the responsibility requirements of Subpart 9.1 of the Federal Acquisition Regulation (FAR). A written notice of award or acceptance of offer, mailed or otherwise furnished to the successful offeror, shall result in a binding contract without further action by either party. Statement of Work Background: The Malcom Randall VA Medical Center at 1601 SW Archer RD, Gainesville, FL 32608 requires annual preventative maintenance on OEM Medrad Injector equipment. Objective: This service contract is for annual preventative maintenance on Medrad injectors located throughout the Diagnostic Imaging Department, Cath Lab and Operating rooms.
