Description

The Southeast Louisiana Veteran Healthcare System (VISN16) has a requirement to locally fabricate and repair artificial limbs for eligible beneficiaries of this Medical Center for the period of January 1, 2013 through December 31, 2013, with four (4) one-year options exercised at the Government's discretion. All work shall be performed in strict accordance with all specifications, statement of work, terms and conditions contained therein. NAICS code is 339113. The corresponding small business size standard of 500 employees or less. This procurement will be a partial small business set-aside. The Government anticipates five (5) awards on a best value basis to meet the needs of Veterans throughout the facility primary service area. A minimum of three (3) of these awards will be made to small business concerns. Electronic version of this solicitation should be available on or about December 14, 2012. Offerors are advised that they are responsible for obtaining amendments. All questions regarding the solicitation must be submitted in writing to the Contracting Officer (Gilda M. Johnson) on or before December 1, 2012 by 3:00 PM local time by e-mail to gilda.johnson@va.gov. Telephone inquiries will not be accepted. Please state the solicitation number on subject line, or e-mail may not be opened. All offers from qualified responsible sources currently providing services in the facility primary service area will be considered. Faxed or e-mailed proposals will NOT be accepted. The following VA forms MUST be submitted with offer and can be found at http://www.va.gov/vaforms; VA Form 2130, Inspection Sheet - Prosthetic Dealer and VA Form 2793, Shop Data Sheet (Artificial Limbs). Award is subject to availability of funds.
