Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, VA260-13-Q-0120, is issued as a Request for Proposal (RFP). The North American Industry Classification System (NAICS) Code for this acquisition is 424210, Size Standard: 100 employees. This procurement is open market small business set aside. ITEM DESCRIPTION OF Isosulfan Blue 1%, 10MG/ML, 5ML injection vile NDC: 67457-0220-05 This solicitation will result in the award of a BPA The BPA will run from the date of award to Sept 30, 2014 Reference the attached RFQ for further details GRAND TOTAL --- ==================== FOB point is Destination: Department of Veterans Affairs, Spokane VAMC, Attn: Janet Anos, 960 General Warehouse Building 14, 4815 North Assembly Street Bldg 14. Delivery shall be made no later than 10 days after receipt of purchase order. Multiple purchase order are expected throughout the year. Award will be made to the offeror representing the best value to the Government. All proposals will be evaluated in accordance with technical capability, compliance with the solicitation requirements and price. All Contractors, both large and small, are required to register in the SAM (system for Award Management) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to https://www.sam.gov/portal/public/SAM/ Quotations must be submitted using the attached RFQ. Quotations are due by Sept 16, 2012, 15:00 PM EST. The assigned Contracting Officer is Paul Richardson.
