Description
NOTICE OF INTENT TO SOLE SOURCE The Indian Health Services, Pinon Health Center, Pinon Arizona intends to award sole source procurement under Simplified Acquisition Procedures to Designs for Learning Inc, 2233 Univrsity Ave W. Ste 450, Saint Paul MN 55114 for the provision of Consultant Services for the Institution. The period of performance for this procurement will be October, 2012 through September 30, 2013. The NAICS Code is 541611. This notice of intent is not a request for competitive proposals. It is not a solicitation or request for offers. Other vendors who believe they can meet the requirement may provide written notification to the contracting office by 3:00pm November 14, 2012. Support evidence must be furnished in sufficient detail to demonstrate the ability of the vendor to supply services that will meet the requirement in a satisfactory manner. Vendor must also provide reference and show competition would be advantageous to the Government. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement in the future. Interested firms must provide a Capability Statement with the following information: Company Name, Address, Point of Contact (POC), Telephone and Fax number, Email Address, Business Size, and Duns number. If no responses are received, the Government will proceed with the sole source procurement to Designs for Learning Inc, 2233 Univrsity Ave W. Ste 450, Saint Paul MN 55114. All potential offers must register in the System for Award Management (SAM) https://www.sam.gov. Item 1: Participant Facilitators The contractor will administer the "We R Family" graduate program. Contractor will utilize graduates of the "We R Family" program to assist in facilitating the various Domestic Violence Preventative Initiative (DVPI) programs. The Contractor will assist in teaching the parenting class, the teen program or the elementary school programs conducted by the "We R Family" or the chapter house programs. The graduates of the "We R Family" program will be compensated at $______ each graduate. Item 2: Grant Writing Trainings The contractor will conduct training on grant writing, grant development, and funding opportunities to counseling service staff, schools, chapter house personnel, community members and the judicial system. There will be a total of three training sessions and assist with one guided grant application process. The class will consist of a minimum of 6 people. Contractor will assist as proofreader for the group. The group will create a database of grant narratives, budget templates, funding resources, and templates to use for future grant writing projects at 250 hours and will equal a total of $_________. Item 3: Family Outreach Projects Contractor will conduct family outreach program and activity projects at 7 chapter houses, at the clinic and at the schools. The program consists of activities such as scrapbooking, card making, Navajo Traditional crafts, knitting, crocheting and various other cultural activities and events to help families develop healthy family unity. Media equipment will be purchase such as; MacBook Pro computer, camera to document for all the activities and to produce digital storytelling and headquarters' virtual site visits $______. Other activities such as Sewing will bring Families Together which is a quilting project that will be used as a therapy tool- sewing supplies and material $_______. Contracted time to teach, therapeutic activities set-up for community projects or/and other therapeutic activities (2.5 hours per event)= 136 hours = $______. Item 4: Training Eye Movement Desensitization and Reprocessing (EMDR) Pinon Health Center Counseling Services therapists have recently been introduced to this therapeutic approach and have found that traumatized patients in particular responded very positively to EMDR. PHCCS therapists who treats trauma will be trained and certified to treat traumas, domestic violence in EMDR. Contractor will make sure that appropriate certification is obtained. The treatment team training - training, materials and equipment $________. Item 5: Advertising for programs The contractor will arrange program advertising and logic with media outlets, such as radio, for the chapter house presentations and other DVPI programs. The Pinon IHS service unit is in a remote location. Radio announcement is an effective means to distribute information within our service area. The radio advertisements will be used to bring more people in to participate in the program. The budgeted amount will vary depending on the topic that will be presented at the chapterhouse. Contractor will arrange program advertising and logic with Radio station for the7 chapter house with 12 presentations. A budget of = $_________total. Item 6: Youth Programs: Contractor will coordinate and administrate the youth programs such as acquiring supplies and materials for Youth programs to facilitate violence prevention activities. These supplies will be used to support youth empowerment and cultural awareness activities. a. Girl/Boy Scout Curriculum - $____ x 30 per youth x 150 Girl/Boy Scout handbooks plus instructor manuals = $_______; There will be camping expedition activities to enhance the theme developed by the leaders for bullying/violence prevention = $________ - Total $ ____+ $_____ + $_______ = $________. b. Dine' Song and Dance Youth Group - Pinon Unified School District ATSA Club - Educational supplies such as regalia, music, and other equipment to use when performing $______. Training and consultants to teach the students the meanings of traditional dress, dance, regalia, and to work with the parents to help them better understand the dances. = $______. The Contractor will assist in administrating the Pinon Community School Youth Traditional singing and dancing group and getting materials for anti-bullying campaign. Contractor will help the school in carrying-out their program and the anti-bullying campaign = $______. Contractor will assist the school in getting their supplies such as regalia, music, and equipment for their performance/program $______. The school may get traditional consultants to teach the students traditional values, conflict resolution and learn traditional song and prays when it's appropriate to student's faith and the meanings of traditional dress, dance, regalia, and to work with the parents to help them better understand the dances Consultant Fee $_____. Total = $______ + $______ + $______ = $_______. d. Adventure Skills Club - Pinon Middle School - development is specifically stated in the PHCCS DVPI Year 3 proposal. e. Through Contractor the Pinon Counseling Services will provide a professional Ropes Course training for the Pinon counseling services staff, some Pinon Unified school staffs, the community school staff and student safety assistants. Contractor will make sure that appropriate certifications are obtained so that the youth ropes course program can start on time this fall. Professional development training $_________. The cost will cover the room, board, travel and per diem and supplies cost which will include safety equipment and other materials to ensure that this training is carried out. The contractor will design the evaluation tool and instrument to carry out the evaluation with the entire DVPI program. The Contractor will analyze the survey data for reports. The evaluation will be used to seek alternative funding to sustain the program after DVPI funding is no longer available. The evaluation will be available to schools, chapter houses and the judicial system to assist in acquiring funding. The evaluation will be conducted on all the DVPI programs and make the data available to DVPI partners $_______ = $_______.
