Description
This Sources Sought is considered market research in accordance with FAR Part 10. No Solicitation is being issued at this time. The purpose of this announcement is to identify qualified contractors capable of providing Prescription Eyeglass Services. There is a need for Prescription Eyeglass Services to support the below listed five (5) Medical Facilities: "Chalmers P. Wylie Ambulatory Care Center, 420 N. James Road, Columbus, OH 43219 "Chillicothe VA Medical Center, 17273 State Route 104, Chillicothe, OH 45601 "Cincinnati VA Medical Center, 3200 Vine Street, Cincinnati, OH 45220 "Dayton VA Medical Center, 4100 W. 3rd Street, Dayton, OH 45428 "Louis Stokes VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106 "The requirement will also support surrounding Community Based Outpatient Clinics (CBOC's) in the Ohio area. The Eyeglasses requirement will consist of the contractor providing prescription eyeglass service to the above mentioned locations. These services will include providing display boards and the equipment and tools required to display frames. The contractor will be required to fit, adjust and repair eyeglasses. The unit price for a single pair of prescription eyeglasses will include all parts, materials, labor, delivery and dispensing services per pair. This includes fabrication, assembly, shipping, handling, lifetime adjustments, eyeglass cases and any related items. The unit price is also to cover minor repairs of glasses and replacement of screws, nose pads and temple parts etc. The contractor shall provide any special lens designs including but not limited to, Balanced Lenses, Prismatic Lenses, Cataract Lenses, Aspheric Lenses, No-Line Bifocal and Progressive Lenses, Trifocal Lenses and Slab Off Prism. Frames should be current style and able to withstand normal wear and tear for one (1) year at no extra charge. All interested vendors are invited to provide information to contribute to this sources sought including, commercial market information. Questions will not be entertained at this time. Respond only to the requested information and questions 1 - 8 below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), HUBZone, 8 (a), Small, Small Disadvantage, Woman Owned Small business, or Large business relative to NAICS 339115, Ophthalmic Goods Manufacturing. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. If not, please register. NOTE: No award can be made to a contractor who is not registered in SAM.
