Description

The Department of Veterans Affairs, VA Network Pacific (VISN 21) intends to negotiate a noncompetitive award with ThedaCare Center for HealthCare Value, Inc, 100 W. Lawrence St. 2nd Floor, Appleton, Wisconsin 54911. This procurement is a sole source award in accordance with FAR 6.302-1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Sole source justification has been prepared in support of this acquisition. This notice is NOT a request for competitive proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-62. The NAICS code is 611430 and the small business size standard is $10.0 million. Scope of Work: The contractor shall furnish all management, supervision, materials, equipment, facilities, transportation and resources necessary to provide educational services and materials in order to implement a network supported initiative in FY13 to develop leaders' understanding and use of daily performance of standard work related to continuous process improvement (CPI) functions at the frontline. The contractor shall independently provide two (2) three-day classroom education and ten days of coaching via telephone and onsite visits for two cohorts of ten (10) participants in the month of January 2013; classroom education to two cohorts of 25 people beginning in February 2013 and ending in August 2013 on how to implement a daily management system taught in six (6) modules with one-month periods of implementation/experimentation between each module; One and one-half day mid-level visit for four cohorts of 15 people to see the system; all program materials for each module and provide property rights of the material adapted for VISN 21. This contract consists of a Base Period (January 1, 2013 through August 31, 2013, date may be adjusted based on date of award). The provisions 52.212-1, Instructions to Offerors (JUN 2008), as modified, applies to this acquisition. Addendum to FAR 52.212-1 includes VAAR 852.270-1 Representatives of Contracting Officers (Jan 2008).
