- STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Failed Power Supply, RF Amplifier and Controller on Open MRI SEG#231118 Local EE# 59249 transaction # WHB-442/673-13-1-6046-0103 2. Scope of Work: The contractor shall provide all resources (labor, materials/parts, tools, equipment, and transportation) necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The vendor shall provide all parts, labor, and test equipment required to troubleshoot a failed power supply, RF Amp and Controller on the Toshiba Open MRI and Restore the system to fully Operational Status. 3. Background The Power Supply has failed on the Open MRI causing a complete systems failure, it is possible that the RF Amplifier and Controller have failed causing the Power Supply Failure. Toshiba will have to troubleshoot the system, replace the power supply and connect then RF Amp and Controller to determine if further parts need to be replaced. 4. Type of Contract: Firm and Fixed Price. B. CONTRACT AWARD MEETING The kick off meeting has been waived and the work can proceed as soon as the purchase order has been issued and provided to Toshiba Medical Systems. C. GENERAL REQUIREMENTS 1.The VA shall not provide service manuals or service diagnostic software to the Contractor. The Contractor shall obtain, have on file, and make available to its FSE(s), all operational and technical documentation (e.g., operational and service manuals, schematics, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name, and/or the manuals themselves shall be provided to the CO upon request. Any charges for parts, manuals, tools, or software required to successfully complete any service required are included within this contract and it's agreed upon price unless specifically stated in writing otherwise. 2.The Contractor will provide all replacement parts required for restore the Open MRI to fully operational condition and within the manufacturer's specifications. All parts must meet or exceed manufacturer's factory specifications to maintain compatibility with systems presently in place and with future performance/reliability upgrades. Parts removed (replaced) by the Contractor become property of the Contractor. 3. At the conclusion of each unscheduled maintenance visit, the Contractor will provide a written service report indicating the dates of service, the model, serial number, and barcode/ID number of the equipment serviced, the name of the service representative, the hours worked, description of the reported problem, and the services performed and parts replaced. The reports will be delivered to COR for signature when work is complete; a copy will be sent to Biomedical Engineering. During non-standard hours, reports will be taken to Police Dispatch for signature. 4. All removable media materials (laptop, DVD/CD, USB drive, etc.) will need to be scanned on site with approved anti-virus by OI&T or Biomedical staff and approved for use prior to the start of any work. D. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E. GOVERNMENT RESPONSIBILITIES The government will make the equipment available in accordance with the established schedule for the work. F. CONTRACTOR EXPERIENCE REQUIREMENTS Each respondent must have an established business, with an office and full time staff to include a "fully qualified" field service engineer (FSE) and a "fully qualified" FSE who will serve as backup. "Fully qualified" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program of the equipment identified. For field experience, the FSE(s) has a minimum of two years experience with respect to scheduled and unscheduled preventive and remedial maintenance of MRI systems. The FSE(s) shall be authorized by the Contractor to perform the maintenance services. All work shall be performed by "fully qualified" competent FSE(s). The Contractor shall provide written assurance to the competency of their personnel and a list of credentials of approved FSE(s) for each make and model the Contractor services at the VA. The Contracting Officer (CO) may authenticate the training requirements, request training certificates, or credentials from the Contractor at any time for any personnel who are servicing or installing any VA equipment. The CO and/or Contracting Officer's Technical Representative (COTR) specifically reserves the right to reject any of the Contractor's personnel and refuse them permission to work on the VA equipment. If subcontractors are used, the CO must approve them in advance. The Contractor shall submit any proposed change in subcontractor(s) to the CO for approval/disapproval.
