- Performance Work Statement 1.0 INTRODUCTION/PROJECT: This contract provides for improved lawn care and grounds maintenance services for Fisher House. The government expects the contractor to use standard industry practices, along with innovative approaches, and active quality control measures to ensure that grounds maintenance is performed in a manner that will maintain healthy grass and shrubs that presents a clean, neat, and professional appearance. The contractor is required to comply with all federal, state and local laws. 1.1 Technical Exhibits: Technical Exhibit 1 - Diagram of estimated grass cutting and flowerbed areas at Fisher House. See attached 1.2 Appendices: Appendix A - Definitions Appendix B - Abbreviations 1.2.1 Sections: 1.0Introduction 2.0Scope of Work 3.0General Information 4.0Reporting Quality Control and Performance Assessment 5.0Performance for Improved Grounds Maintenance 6.0 Hours 7.0 OSHA/ Quality 1.2.2 Licenses, Permits and Bonds: The Contractor shall provide at his own expense all licenses, permits, and bonds as required by the State, Counties, and Municipalities through where the Contractor will operate in the transport and disposal of the solid waste removed under the terms and conditions of the contract. 2.0 SCOPE OF WORK: Contractor shall provide all material, labor and supplies required to provide lawn service to 5 acres at the Fisher House weekly, which includes: "Cut grass weekly "Edge walkways, driveways and bed lines with a steel blade edger "Weed eat areas as needed "Blow curb areas and parking lot area as needed "Keep weeds out of beds, curb, and parking lot areas with round-up "Prune shrubbery as needed to maintain a uniform shape "Pick up trash in common areas "Provide pine straw or mulch once a year "Leaf removal from property in fall/winter seasons "Haul off all debris from property 2.1 Turf Maintenance: A. Mowing and Trimming Lawn Areas 1.Remove all trash and debris from lawns prior to each mowing. 2.Maintain lawns to give a neat, uniform appearance and frequently enough to prevent visible accumulation of grass clippings. Remove any visible accumulation of clippings by sweeping or blowing. 3.Adjust mowing frequency to the growth rate of the turf. Do not remove more than one-quarter to one-third of the total leaf area at any one mowing. Vary the direction of mowing each time lawns are mowed. 4.Mow lawns when grass reaches a height of 2 ½ inches and cut to a minimum height of 1 ½ inches. Mowing height may be increased 2 inches during the summer. 5.Keep mowers sharp and properly adjusted during cutting operations. 6.After each mowing, trim lawn edges to the same height as adjacent turf. Where adjacent to sidewalks, curbs, etc., neatly edge lawns weekly as needed. Do not line trim within 2 inches of tree trunks, plant foliage or other non-durable surfaces; hand trim areas where power trimmers are not to be operated. After completion blow off all sidewalks, roadways and parking lot and patios B. Weed Control 1.Keep all flower and shrub areas free of weeds through timely use of both cultural and manual labor control procedures. C.Spring Reconditioning 1.Rake all lawns prior to initiation of active spring growth to remove dead grass, twigs, leaves, and other debris. 2.Roll lawns, if required, to smooth uneven areas caused by heaving action of winter freezing and thawing. Roll only during very early spring after soil has thawed. 3.If thatch exceeds one-half inch depth or soil compaction is evident, aerate lawns after soil thaws and turf enters the active growing period. Aerate using a spoon or core method. If core type operation is employed, use a drag or other approved method to break up cores. 2.2 TREES, SHRUBS, AND GROUND COVER A.General 1.Keep all beds and planters in a clean, well-groomed, weed- and pest-free condition. Remove trash and debris from beds and maintenance edges weekly. 2.During fall and winter season, all fallen leaves are to be collected and remove from property on a weekly basis. B.Cultivating 1.Cultivate all bed areas as often as necessary to keep them in a loose, uniform condition and free of weeds. 2.Maintain a symmetrical ring of cultivation around tree trunks. 3.Cultivate to a depth of 2 to 3 inches, leaving the soil at a uniform grade. 4.Do not cultivate around surface-rooted broadleaf evergreens which should not be disturbed other than by hand-weeding. 5.Cultivate at least once a month during the growing season, prior to irrigating during the winter, and after fertilizing. 6.Cultivate mulched beds prior to the seasonal addition of new mulch materials. C.Mulching 1.Top-dress all shrub beds and tree rings with shredded composted bark at least once a year during April to maintain a uniform depth of approximately 2 inches. Where mulch exceeds 3 inches, remove excess before topdressing. 2.Apply 1 inch of double screened, finely shredded bark to all bulb plantings in the late autumn when seasonal flowers are removed. D.Edging 1.Maintain all steel edging at 1 inch above finish grade. 2.Keep all hand-cut bed lines and tree rings cleanly edged, with smooth continuous curves or straight lines as shown on the Key Plan. Edges shall be cut at least bi-weekly during the growing season, and any encroachment of Centapede lawn removed from the beds. E.Weed Control 1.Keep planting beds, stabilized aggregate surfaces, and tree rings within the improved grounds areas free of weeds by cultivation and manual removal. F.Insect/Disease Control 1.Inspect all plantings on a weekly basis for possible infestations report all findings to COR. G.Pruning 1.Prune all trees and shrubs as specified herein. Work shall be accomplished by arborists and horticulturists experienced in all aspects of pruning. 2.All pruning shall conform to standards established by the National Arborist Association. 3.In general, do not shear plants; remove individual limbs back to main trunk or leader. Do not leave stubs. Use proper tool for cut being made. Keep cutting tools sharp and clean. Disinfect tools between cuts when pruning diseased or easily infected plants. Be familiar with and maintain natural habit of plants when pruning. 4.Utilize the following procedures when pruning the plant categories listed below: a.Shade and Ornamental Trees: 1.Maintain natural habit and proportions. Remove dead, damaged, or dangerous branches, water sprouts, suckers, and crossing branches. 2.Major Storm Damage repair is not a part of this contract. b.Upright Conifers: 1.Remove dead or damaged limbs and multiple leaders. Maintain compact, uniform growth by selectively removing portion of young 'candles' during extension in the spring. c.Spreading Conifers: 1.Remove dead or damaged branches. Do not shear; prune groups or masses as a single unit, not individual plants. d.Deciduous Shrubs: 1.Prune to maintain natural habit and proportions; do not shear. Periodically, thin canes to maintain vigor and new growth. 2.Time pruning to prevent removal of flower buds. e.Hedges: 1.Shear hedges at least twice annually, once after spring flush, and once in late summer. 2.Maintain size and proportion approved by Owner. Bevel face of hedges to optimize exposure to light. 3.Conduct rejuvenative pruning as directed by the Owner. f.Ground Cover: 1. To encourage dense, compact growth, shear Asian Jasmine during early spring when plants are still dormant. g.Annual Flowers: 1. Pinch back plants as required to maintain uniform, compact plants. Pinch off (dead head) spent blooms weekly. H.Plant Material Replacement 1.Replace all dead or damaged plant materials that cannot be restored to original quality and character. 2.Make plant replacements only during normal planting seasons. 3.Replace all plant material in kind and, where possible, size; and in no case smaller than specified on original landscape documents. Plants shall conform to American Standard for Nursery Stock ANSI Z60.1. 3.0 PERSONNEL: The Project Manager shall be responsible for overall quality control management. PM may be required to attend scheduled and unscheduled meetings or inspections as required, at no additional cost to the Government. 3.1 Contractor personnel shall meet the following requirements: 3.2 Contractor Managers and Crew Leaders shall be legal residents of the United States and shall able to read, write, speak, and understand English. 3.3 Contractor employees shall be legal residents of the United States, be qualified, possess the skills, knowledge, and training to satisfactorily perform the services required by the contract requirements to include being physically able to do assigned work. 3.4 Contractor personnel performing work under this contract shall not be considered employees of the Government. 3.5 Contractor personnel operating a vehicle in performance of this contract shall have current, valid driver's licenses, vehicle registration, and proper auto insurance coverage. 3.6 Contractor personnel shall remain fully clothed at all times while performing work under this contract. 3.7 The Contractor shall immediately remove from the job site any employee for misconduct or security reasons. 3.8 The removal from the job site of such a person shall not relieve the Contractor of the requirement to provide sufficient personnel to perform adequate and timely service. 4.0 REPORTING: For services performed during normal working hours, the contractor's technician is to report to the Grounds Supervisor (Chuck Jones ext. 6051), to report/sign-in and obtain an identification badge, which shall be worn at all times while the technician is on station. 4.1 After work is completed, the contractor's technician(s) are to submit in writing, a complete report of services performed for each item of equipment. The Grounds Supervisor, or his designee, must sign the service report before the technician(s) leave the facility. 5.0 PERFORMANCE: Contractor shall follow safety precautions, including the use of personal protective equipment (i.e. proper attire, masks, gloves, and eye protection). All tools shall be fully supervised by contractor technicians and fully secured when not in use. 5.1 Once work is completed, contractor is to clean up and haul off all debris from property and leave the area of work ready for hospital business. 6.0 HOURS OF WORK: Work is to be performed during normal duty hours, 7:30am - 4:00pm Monday thru Friday and shall be completed no later than 12/31/2013, without prior approval from the VA. 6.1 FEDERAL HOLIDAYS Federal Holidays are listed below. Should a Federal Holiday fall on a Saturday, the Friday immediately before is considered the holiday; if the Federal Holiday falls on a Sunday, the Monday immediately following is considered the Holiday. New Year's DayLabor Day Martin Luther King's BirthdayColumbus Day Presidents' DayVeterans' Day Memorial DayThanksgiving Day Independence DayChristmas Day 7.0 PERSONNEL SAFETY The Contractor shall be responsible for assuring equipment is in a safe and operational condition and shall perform all tasks in a safe, responsible manner. 7.1 The Contractor shall perform work requirements in a manner which protects personnel within the area from any harm or injury. 7.2 The Contractor or their employees shall immediately notify their supervisor of any accident requiring emergency medical treatment. 7.3 Contractor Caused Damages: The Contractor shall be responsible for all damages to Government property occurring as a result of the Contractor/contract employees' work. Contractor must exercise care and due diligence when performing grounds maintenance services, and not to project any foreign objects that may cause injury to children, staff members, visitors or other pedestrians/personnel; or damage, scratch, or break school's windows, doors, structure, etc. This is also applicable to any automobiles that may be parked, or moving throughout/within the school's areas. 7.3.1 The Contractor shall notify the COR no later than the next work day of any damages caused by the Contractor employee's or equipment. 7.4 The Contractor shall make all necessary repairs, at no cost to the Government, and to the satisfaction of the Government. 7.5 Quality Control 7.5.1 The Contractor shall establish and maintain a complete Quality Control Plan to ensure the requirements of the contract are provided as specified. Technical Exhibit 1: Ground Maintenance Areas at Fisher House Total Property: approx. 4 Acres Sidewalk & Patio: Approx. 3,900 sq ft. Parking Lot & Roads: Approx. 20,000 sq ft. Flowers Bed & Shrub: Approx. 40,000 sq ft area Lawn (Grass Area): Approx. 82,000 sq ft. Appendix A: Definitions. Acceptable Quality Level (AQL): The AQL is the maximum percent defective (or the number of defects per hundred units) that, for purposes of sampling inspection can be considered satisfactory. Contracting Officer (KO): A person duly appointed with the authority to enter into and administer contracts on behalf of the Government. Contract Discrepancy Report (CDR): A formal, written documentation of Contractor nonconformance or lack of performance for contract work. Contracting Officer's/Designated Government Representative (COR/DGR): An individual designated by the Contracting Officer as their representative administering a contract. The source and authority for a COR is the Contracting Officer and limitations of a COR are contained in the written letter of designation. Customer Complaint: A method of documenting certain kinds of contract service situations which is used to evaluate Contractor's performance. Defective Service: A service output that does not meet the standard of performance associated with it in the requirements of this contract. Emergency Response: A call that is life threatening or could cause harm to individuals or property damage, (Example: Broken gas line, broken steam or water line, or electrical hazards). Facility: Building or Office (Real Property). Government Property: All property owned by or leased to the Government or acquired by the Government under the terms of the contract. Government property includes both Government-furnished property and Contractor acquired property as defined in FAR 45.101. Mandatory Documents: Directives with which the Contractor shall apply. Other Facilities: Facilities such as temporary and permanent outlining buildings (i.e. guard shacks/access control points). Performance Assessment: Those actions taken by the Government to assure services meet the requirements of the Performance Work Statement (PWS) and all other service outputs. Performance Assessment Plan: An organized written document used for performance assessment. The document contains specific methods to perform surveillance of the Contractor. Performance Requirement: The point that divides acceptable and unacceptable performance. In the case of surveillance by random sampling, the performance requirement is the maximum number of defectives in the random sample chosen that may occur before the Government will affect the price computation system in accordance with the PRS Summary and Inspection of Services clause. When the method of surveillance is other than random sampling, the performance requirement is the number of defectives or maximum percent defective in the lot before the Government will affect the price computation system in accordance with the PRS and the Inspection of Services clause. Performance Requirements Summary (PRS): Identifies the key service outputs of the contract that will be evaluated by the Government to assure contract performance standards are met by the Contractor. Planned Sampling: A sampling method in which work outputs are selected in accordance with subjective criteria established in the surveillance plans. Quality Assurance Evaluator (QAE): A Government person responsible for assessment of Contractor performance. Quality Control: Those actions taken by a Contractor to control the performance of services so that they meet the requirements of the PWS. Random Sampling: A sampling method where each service output in a lot has an equal chance of being selected for quality assurance surveillance. Recycling: A process which by post-consumer waste that would otherwise become refuse is collected, separated, and/or processed and returned to the economic mainstream in the form of raw materials or products. Sample: A sample consists of one or more service outputs drawn from a lot for performance assessment. Yard Waste: Includes grass cuttings, tree trimmings, stumps, leaves, limbs, and similar waste material.
