- The Department of Veterans Affairs, Charlie Norwood VA Medical Center, Augusta GA is considering awarding a sole source contract to D&N Dental Designs, Inc, 3976 Columbia Rd, Martinez GA 30907, for: SCOPE OF WORK: The contractor shall fabricate fixed prosthodontic dental prostheses in strict accordance with the dental laboratory order submitted by the VA care provider. This shall include but not be limited to the following: all fabrication aspects of crowns, fixed partial dentures, custom implant abutments, implant supported prostheses, all porcelain crowns, and porcelain veneers. The normal turnaround time for fabrication of the fixed prostheses will be fourteen (14) working days (Monday through Friday). In addition they must be able to provide a one day turn around on repairs/adjustments such as porcelain modification and adjustment of proximal contacts. Materials used will be in accordance with the requesting dental care provider. The following materials shall be used when otherwise not specified: a.All-metal fixed partial dentures and crowns - High Nobel Alloy: Jensen JCB alloy or equivalent b.PFM Metal - High Nobel Alloy: Olympia or equivalent c.Implant abutments - High Nobel Alloy, titanium or procera zirconia d.Implant systems - Nobel Biocare System or equivalent The contractor shall provide the following services at the VA Dental Service: a.Provide forms at the VA dental laboratory for the authorization of work at their facility. b.One daily pick-up and delivery Monday through Friday at the VA Dental Service to include but not limited to the impressions, diagnostic casts, and all laboratory necessary materials relating to fixed prosthodontic care. Contractor shall consult with the appropriate dental care provider at the CNVAMC regarding any specific laboratory order. CNVAMC is responsible for keeping the contractor informed of their progress in meeting these performance measures. This will be the responsibility of the Dental Department, CNVAMC, Augusta, Georgia. Neither the beneficiary, his/her insurer, nor any other third part shall be billed. The Chief of Dental and/or COTR will monitor performance through government information systems and records, patient records, customer service information, and contractor reports. Laboratory: Preferably within 30 miles of the VAMC and with a one day turnaround time on repair cases. Laboratory must be veteran owned. Dental prostheses must be made locally and not be shipped out of country for fabrication per VHA Handbook 1130.01. BACKGROUND: The uptown (CNVAMC) is located on One Freedom Way, off of Wrightsboro Road three miles from the Downtown CNVAMC on 15th Street. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals or a solicitation for offers. However, parties interested in responding to this notice shall submit technical data sufficient to determine capability in providing the same product to Shirley Adams, Contract Specialist at Shirley.Adams@va.gov or fax (478-274-5588 by December 11, 2012, no later than 2:00 PM EST. All capability statement received by closing of this notification of this synopsis will be considered by the Government. A determination by the government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This acquisition is being conducted under simplified acquisition procedures. This requirement shall be set aside for veteran owned small businesses. The intended procurement will be classified under NAICS 339116 with a Small Business Size Standard of 500 employees. This is for informational purposes only and not to be construed as a commitment by the Government. Any information submitted to this notice of intent is voluntary. Your responses to the information requested will assist the Government in determining the appropriate acquisition method including whether a set aside is possible.
