Description

PROVIDE ALL EQUIPMENT AND SOFTWARE FOR THE VETERANS HEALTHCARE SYSTEM OF THE OZARKS (VHSO) LOCATED IN FAYETTEVILLE, ARKANSAS. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All offerors submitting an offer must be registered on www.sam.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA256-13-Q-0182. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-61. (IV) This combined solicitation/synopsis is for purchase on the following commercial items: Item # 0001 Microscope : Konan model CellChek XL Tough System, or equal Quantity (1) each Item # 0002 Konan Cellscreen software V.1.0.4. WIN TV, or equalQuantity (1 ) each (V) Description of requirements is listed below. Complete system must have the following features/capabilities: * Delivers cellular level imaging of the corneal endothelium with the industry's most comprehensive tools for cellular morphology and trending analysis. The microscope system includes 19" touch screen motorized table, and printer. * System must provide for the patient and the tech sitting on the same side of the machine. * Auto focus, auto alignment, auto image capture needed * System automatically records the exact location of the data capture, so data can be compared over time. * Need central and peripheral image capture * Trends analysis capability, recording data location with each data sample. Providing the ability to compare same tissue areas and trending of cell counts and morphology over time. *Provides for "Center Method" (semi-automated). Providing capability for analyses for corneas affected by disease, dystrophy or trauma having few cells that can be analyzed. Must have the semi-automated method vs. the fully automated methods. (VI) Place of delivery is Veterans Healthcare System of the Ozarks, VA Medical Center, Warehouse, 1100 N. College Avenue, Fayetteville, Arkansas 72703. (VII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Feb, 2012), applies to this acquisition. (VIII) Award will be made to the offeror whose offer, meeting the requirement of this solicitation, is most advantageous to the Government. (IX) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2012), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (X) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Feb, 2012), applies to this acquisition. (XI) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Aug, 2012) applies to this acquisition. The following clauses under subparagraph (b) apply: (39) 52.225-1, Buy American Act-Supplies (Feb, 2009) (41 U.S.C. 10a-10d). (42) 52.225-13, Restrictions on Certain Foreign Purchases (June 2008) (48) 52.232-33, Payment by Electronic Funds Transfer-Other Than Central Contractor Registration (May 1999) (31 U.S.C. 3332). Questions must be received no later than 12:00PM CST, December 11, 2012. (XIV) Quotes are required to be received in the contracting office no later than 4:00 P.M. CST on December 14, 2012.
