- This is a request for proposal for electrical and fence grounding upgrades at the Remote Communications Link Repeater, Stafford, KS. Vendor will provide all materials, supervision, labor and equipment to perform all work according to the attached Specifications. Site is located in Stafford County, Kansas. Projected start date is 5 days after Notice to Proceed. The FAA will not pay for any information received or costs incurred in preparing the response to the request for proposal. Therefore any cost associated with the request for proposal submission is solely at the interested vendor's expense. A PRE-PROPOSAL SITE VISIT IS SCHEDULED FOR JANUARY 9, 201. AT 10:00 A.M. CST. SEE PAGE 31, SECTION L, 3.2.2.3-63 SITE VISIT (CONSTRUCTION). CONTACT LO XAO KIATOUKAYSY, 620-662-5150 OR 620-200-4655 (CELL), OF YOUR INTENT TO ATTEND BY JANUARY 8, 2013 AT 2:00 P.M. CST. THIS IS THE ONLY SITE VISIT THAT WILL BE HELD. The site is approximately seven (7) miles north of the town of Sylvia, KS, located on 17th Avenue, between north Brownlee Road and north Sylvia Road. There is no street address. The lat/longs are 38 04'18.96"N 98 25'34.65"W. The competition will be based on lowest price / technically acceptable. This competition is open only for small businesses, or service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SBA for participation in the SBA's 8(a) Program. Contractors must be registered in the System for Award Management (SAM) database prior to submitting a response. Vendors participating in the SBA's 8(a) Program must provide a copy of their SBA 8(a) certification letter. In order to make this determination the FAA requires the following from interested vendors, which will be used in the evaluation process: 1. Pricing. 2. Evaluation factors: - The contractor must provide references/locations of previous projects. - The contractor must provide workforce qualifications of key employees (on-site supervisor and technician / journeyman). Attached are the Specifications, Terms and Conditions, Business Declaration Form and Contractor's Release Form DOT F 4220.4. Please respond by reviewing these items, and submitting a completed, signed pricing sheet, completed Section K Representations, Certifications, and Other Statements of Offerors, the requested evaluation information and a filled-out and signed copy of the Declaration form (please submit electronically, email, to address below). All responses to this request for proposal must be received by THURSDAY, JANUARY 16, 2013, 2:00 PM CST (Oklahoma Time).
