Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The MICC-Ft. Dix Contracting Office at 5402 1st Avenue, Ft. Dix, NJ 08640 has a requirement to provide Road Salt and combined Road Salt/Sand for delivery to Ft. Devens, MA. The anticipated solicitation number shall be W911S1-13-T-0030. This requirement is for a maximum of 600 Tons of each material to be delivered in increments of approximately 25 Tons each within two hours of notification of the delivery need. The resulting contract will cover a 12 month period, but that is not to be interpreted as deliveries to occur in each month, nor that will it require 12 months to reach the contract's limit. It has been determined the most appropriate NAICS Code for this requirement is 327999 - All Other Miscellaneous Nonmetallic Mineral Product Manufacturing which has a small business size standard of 500 employees. The response date shall be 26 December 2012 at Noon with responses sent to robert.i.knight.civ@mail.mil. Award is anticipated to take place by 10 January 2013. Provide quotes for items listed below with the following information: Item# - Description - Quantity - Unit of Issue - Unit Price - Total Cost 0001 - Road Salt & Sand MIx - 600 - Dry Ton 0002 - Road Salt - 600 - Dry Ton The folowing provisions are included by reference: 52.204-99 System for Award Management (SAM) Registration 52.212-1 Instructions to Offers - Commercial Items 52.212-3 Reps & Certs 52.215-1 Instructions to Offerors - Competitive Acquisition 52.252-1 Soliciattion Provisions Incorporated by Reference 252.225-7031 Secondary Arab Boycott of Israel 252.209-7999 Rep by Corp Regarding Unpaid Delinquent Tax or Felony Conviction Identification of Contractor's Employees - RAPIDGate Program.* a. Each employee who requires access to Fort Devens to perform work under any contract, at any tier, must obtain a RAPIDGate identification badge to obtain access to Fort Devens. A RAPIDGate identification badge will only be issued to an employee, at any tier, if the employee requires access to Fort Devens more than twice per week or when the contract spans more than 30 days. Contractor employees, at any tier, who require access to Fort Devens twice per week or less often. The visitor's pass will be issued for a maximum of 1 day at a time. A RAPIDGate identification badge will only be issued to contractor employees if the contractor participates in the RAPIDGate Program. The RAPIDGate Program is a mandatory program. b. The contractor shall provide information as required by the RAPIDGate Program Contractor (RPC) to enable the contractor to conduct a criminal history background check (CHBC) on contractor employees who are to have access to Fort Devens. The contractor is responsible for paying the RPC fee per employee for conducting the CHBC and issuing the RAPIDGate identification card. If the CHBC is not adverse, a RAPIDGate identification badge will be issued by the RPC to the contractor employee. If the CHBC is adverse, the RPC is prohibited from issuing a RAPIDGate identification badge to the contractor employee. If a RAPIDGate identification badge is denied for any reason, the contractor employee may only enter the installation by obtaining a visitor's pass; however, if the contractor employee does not meet the criteria for being issued a RAPIDGate identification badge, the Government may, in its sole discretion, decide not to issue a visitor's pass to the contractor employee. c. If an employee no longer needs an identification badge for any reason (e.g., quits his/her job or no longer performs work under the contract), the contractor shall return the identification badge to the Police Station with two (2) calendar days of such change. If the identification badge cannot be returned with the required time from for any reason, the contractor shall immediately notify both the RPC and the Police Station verbally, followed up in writing the next work day. Email notification is authorized. An employee's inability to obtain entrance to a Government installation because he/she does not have the required identification badge or visitor's pass shall not excuse timely performance of the requirements of this contract. Badges are currently issued at the Fort Devens Police Station. The Government may change the location at which identification badges are issued or returned, with or without advance notice to the contractor. Any such changes shall not be a basis for adjusting the contract price under the clause of this contract. d. RAPIDGate identification badges shall not be reproduced or copied by the contractor, its subcontractors, or their employees. If an employee's identification badge is lost, stolen or reproduced, the contractor shall verbally report loss, theft or reproduction to both the RPC and the Police Station on the day such loss, theft, or reproduction is discovered, followed by a written report of the circumstances to both the RPC and the Police Station within one (1) calendar day after the loss, theft, or reproduction is discovered. e. Each contractor employee shall wear the RAPIDGate identification badge while performing work under the contract. The identification badge shall be worn on the upper front of the outer garment unless precluded by OSHA regulation (s). The identification badge shall not be used for access to any Government installation except for performance of work under the contract for which it was issued. f. The contractor shall, upon expiration or termination of the contract, collect all identification badges and turn them in to the COR.
