The Fort Belvoir Community Hospital Commander is responsible for religious, spiritual, moral, and ethical activities of all personnel within the command on an equitable basis. The chaplain designated as DPC Chief, Fort Belvoir Community Hospital has primary staff responsibility to assist the command in the planning, development and implementation of these activities. The chaplains, chaplain assistants, religious program specialists, civilian employees, and non-personal services aid the Commander in this task. 1.1.1 SCOPE OF WORK. The contractor shall provide the necessary personnel, equipment, supplies, facilities, transportation, tools and materials to perform as defined in this SOW. 1.1.2 NON-PERSONAL SERVICES. This contractor is a non-personal services contract under which the personnel rendering the services are not subject, either by the contract's terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. 1.2 COMMUNICATION. The Contractor's employee shall be able to read, write, understand and speak English clearly, to effectively communicate with government employees. 1.3 Physical Security. The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured. 1.4 Personnel Standards Requirements. The Contractor shall provide contract Imam(s), fully accredited and endorsed by the ISNA to perform religious services as required by the terms and conditions of this contract. The Government reserves the right to give final approval of all Imams proposed to serve under this contract, based on qualifications as set forth in paragraph 1.4 and 1.5. The Contractor will determine the number of Imams necessary to fulfill the obligations of this contract. 1.4.1 The Contractor shall provide personnel capable of all work specified herein. 1.4.2 The Contractor shall provide a replacement when contract Imam(s) (1s)(are) absent. Substitutions must meet the same qualifications as contracted Imams, as prescribed in paragraph 1.4 and 1.5. 1.4.3 The Contractor shall provide for the direct, daily supervision of contract personnel, and shall act as a liaison between the Contracting Officer, Contracting Officer's Representative (COR) and the contracted Imams and/or substitute Imams. 1.4.4 The Contractor shall provide a list of Imams and support personnel, and resumes for all. During the contract term, the contractor shall notify, and submit resumes to, the COR for any new or substitute personnel and any changes in existing personnel resumes. Ordinarily, such notices shall be provided to the COR one week In advance of the substitution. 1.4.5 The contractor shall provide to the COR, in writing, a point of contact (POC) and phone number to place calls for services within five (5) days of Award. At anytime during the performance of the Award the POC and/or phone number changes, the contractor shall notify the COR, in writing, within 24 hours of the change. 1.5 TASK. 1.5.1 Contractor must possess ISNA endorsement, dated within the past 12 months. For the purpose of this standard, ISNA endorsement is a written official statement by their national endorsing body, certifying that the contractor/Imam Is in good standing with their group/organization; and stating that the individual is, In the opinion of the endorsing body, qualified to perform the full range of ministry required in the military pluralistic setting. 1.5.2 Contractor must possess equivalent educational qualifications to be endorsed by the ISNA. 1.5.3 Contractor must meet or exceed requirements of the Joint Commission; Contractor may possess units of verifiable Clinical Pastoral Education (CPE) as defined by the Association for Pastoral Education (ACPE) or its equivalent as determined by the Chief, Department of Pastoral Care, FBCH. 1.5.4 Contractor and contract Imam will be experienced in providing religious care in a hospital environment to patients, staff, and family members. 1.5.5 Contractor and contract Imam must possess a knowledge and ability in all the areas associated with the tenets of the Islamic faith. 1.5.6 The Contractor shall plan, coordinate, and provide religious care/ministrations to include weekly patient visitation and religious care, as required and requested. " Patient Visitations " are scheduled visits to patients and family members with special emphasis on bedside visits in areas such as inpatient wards, intensive care units and the Emergency Department. Religious care/ministrations are defined as comprehensive and in depth ministry provided for patients, families, and staff within the framework of their religious faith, to include religious counseling and professional consultation, as well as responding to mass casualties/emergencies/alerts. Contractor will ensure that all visitations are appropriately charted in patient medical charts. 1.5.7 The Contractor will plan, coordinate and provide ministrations, as required and requested, including: Muslim services conducted in the Hospital Chapel (FBCH). Other ministrations shall be performed as required in accordance with the norms of the Muslim practice. Such ministrations shall be recorded. Muslim worship services are as follows: 1.5.8 Friday Worship Service (Ref AR 165-1). The Contractor will perform, as required and requested, one (1) Muslin worship service each week for fifty-two (52) weeks, for a total of 52 units. 1.5.9 Holy Day Worship Services (Ref AR 165-1). The Contractor shall perform, as required and requested, one (1) worship service per each Holy Day. 1 Day Per Week (Friday) (52 Units) 1.5.10 The Contractor shall be available and provide as required and requested, " on-call " religious ministrations on a 24- hour basis. Contractor will respond to all "on-call" requests that take place before, during, or after "normal duty hours," as soon as possible. 5 Calls Per Month (60 Units) for Islamic Patients. 1.5.11 Contractor and contract Imams will not accept fees for performing ministrations, and ordinances, which are performed and remunerated under the terms of this contract. Gifts and honorariums shall be deposited in the FBCH Chaplains' Fund. 1.5.12 "On-call" is defined for the purpose of this contract as the 24-hour availability and response of a contracted Imam to provide religious ministrations. 1.5.13 "Normal duty hours" are for the purpose of this contract defined as duty performed by the contractor between the hours of 8:00 AM to 5:00 PM EST, excluding Federal Holidays Monday through Friday, (5) five days a week. 1.5.14 "Unit of Basic Religious Ministrations " is an intentional visit to areas or persons designated by the COR for patient visitation and religious care, or attendance and participation In the National Prayer Breakfast, Thanksgiving Service and other special ceremonies and events, as requested. 2 Days Per 52 Weeks (104 Units) 1.5.15 " Special Ministrations," for the purpose of this contract, include funerals, memorial services, weddings and other events as requested or scheduled by the COR. Contractor will receive one unit per each "Special Ministrations" activity requested to perform. 2 Events Per Month (24 Units) 1.6 PERIOD OF PERFORMANCE. The period of performance shall be for one (1) Base Year of 12 months with an additional four option years. 1.7 PLACE/HOURS OF PERFORMANCE/NORMAL BUSINESS HOURS. The Place of Performance is located at FORT BELVOIR COMMUNITY HOSPITAL, 9300 DeWitt Loop, Fort Belvoir, VA 22060. Monday - Friday, between 8:00 AM and 5:00 PM EST, excluding Federal Holidays. 1.8 HOLIDAYS. The following Federal Holidays are observed by FBCH: New Year's Day, January 1st Martin Luther King's Birthday, 3rd Monday in January President's Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veteran's Day, November 11 h Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th NOTE: Any of the above Holidays falling on a Saturday will be observed on the preceding Friday; Holidays falling on a Sunday will be observed on the following Monday. Any Holidays that are declared by Presidential Executive Order shall be observed in the same manner as the Holidays listed above. If the area in which a contract employee is scheduled to work is closed due to the Holiday and the employee is not required to report in, payment will not be made for those hours. Closures of the installation due to inclement weather or other such acts of God shall be handled in the same manner. 1.9 CONTRACTOR PROVIDED SERVICES. The Contractor shall furnish all personnel and services to comply with the requirements of this contract. 1.10 GOVERNMENT RESPONSIBILITIES. The FBCH shall provide the Contractor with appropriate FBCH identification cards and vehicle registration(s). 1.10.1 The FBCH shall provide the Contractor the funding necessary to be authorized to perform marriages within the Commonwealth of Virginia, pursuant to the Code of Virginia at the expense of the FBCH Chaplain's Fund. 1.10.2 The FBCH shall provide the Contractor with supplies to include an area for religious ministrations. 1.10.3 The on-call Duty Chaplain will initiate all "on-call" requests for Muslim religious ministrations. Contract Imam will report time, date and patient responded to for Duty Chaplin to record in the DPC Chaplain Duty Log. The Contractor will receive one unit per each "on-call" response provided. 1.11 QUALITY CONTROL PLAN. The contractor shall develop and maintain an effective Quality Control Program (QCP) to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor's QCP is the means by which he assures himself that his work complies with the requirement of the contract. The contractor shall deliver to the COR a QCP within 30 days after contract award. If the QCP changes during the performance of the contract, the contactor shall submit to the COR, the changed QCP within five (5) business days after changes occur. 1.12 QUALITY ASSURANCE. The government shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance and the minimum acceptable defect rate(s). 2.0 DEFINITIONS/ACRONYMS. • CONTRACTING OFFICER (KO): A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings on contracts. • CONTRACTING OFFICER'S REPRESENTATIVE (COR): A Government employee, appointed in writing and within the limits of authority as delegated by the contracting officer, authorized to act as his representative in monitoring a contract. • FORT BELVOIR COMMUNITY HOSPITAL (FBCH): Hospital where the Services will be performed. • MILITARY TIME Is on a 24-hour clock, i.e. 8:00 a.m. 0800 9:10 a.m. 0910 Noon 1200 8:00 p.m. 2000 Midnight 2400 12:01 a.m. 0001 MILITARY DATES are written by placing the day of the month, followed by the first three letters of the month, followed by the last two digits of the year, i.e., September 2, 2001 - 02 Sep 01 November 10, 2004 - 10 Nov 04 May 1, 2005 - 01 May 05 • MILITARY TREATMENT FACILITIES: A military hospital or clinic. • QUALITY ASSESSMENT AND IMPROVEMENT (QA&I): An ongoing program designed to objectively and systematically monitor and evaluate the quality and appropriateness of patient care, to pursue opportunities to improve patient care, and to resolve identified problems. • QUALITY CONTROL: Those actions taken by the Contractor to control and ensure that performance of duties meet the requirements of the contract. • TRAINING HOLIDAYS: Training holidays are days designated by the MTF Commander either the workday preceding or following a legal public Holiday. The number of Training Holidays may vary from year to year. 3.0 APPLICABLE TECHNICAL ORDERS, MANUALS, REGULATIONS, DIRECTIVES AND FORMS. All applicable directives and publications {advisory or mandatory), and supplements or amendments to these directives and publications will be current when furnished to the Contractor. Directives will be available to Contractor upon request. Current issues of many DA publications and many forms can be accessed at httq://www.usaqa.armv.mil. DOD publications and forms can be accessed at htto://www.defenselink.mil. 3.1.1 Publications and forms not on the websites can be obtained from the MTF. 3.1.2 The Contractor is required to follow all mandatory documents to the extent they apply to this contract. Any such changes to mandatory publications that cause a change in the scope of performance will not be implemented by the Contractor until the Contracting Officer issues a modification in writing. 4.0 ATTACHMENTS. ATTACHMENT 1 PERFORMANCE REQUIREMENT SUMMARY The Government will measure the Contractor's performance under the performance objectives listed in the following table by applying the corresponding performance standards and acceptable performance quality levels. Other performance evaluation factors that will be monitored and documented in the Contractor's performance records but cannot be quantified by numerical measurements: Contractor providing personnel exceeding the minimum qualification standards; Performance above or below the standard, for above factors, will be documented in the Contractor's Performance Assessment Reporting System (CPARS) for the reporting period.
