- Certification Form SPECIFICATION - PERMALIFE NATIONAL ARCHIVES AND RECORDS ADMINISTRATION REQUEST FOR QUOTES (4,000 REAMS) PERMALIFE BOND PAPER (BRAND NAME ONLY) NATIONAL PERSONNEL RECORDS CENTER. ST LOUIS, MO NAMA-13-Q-0006 This is a combined synopsis/solicitation for commercial supplies in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quotation number is NAMA-13-Q-0006. This Request for Quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-63 effective DEC 10, 2012. The NAICS Code 322121 Paper (Except Newsprint) Mills (750 Employees), 424120 Stationary and Office Supplies Merchant Wholesalers (100 Employees), and 322230 Stationary Product Manufacturing (500 Employees) are applicable. The requirement is a 100% small business set-aside and the Small Business Size Standard is as above. All responsible small business sources may submit a proposal that will be considered by NARA. Quotations will be evaluated IAW FAR 12, 13 and 6.302. The contractor must have a Dunn and Bradstreet Number (DUNS) and be registered in the Central Contractor Registration (CCR). Award will be base on lowest priced technically acceptable (LPTA) offer. This will be a firm-fixed price purchase order. The purpose of this contract is to obtain Permalife Bond Paper for the National Archives and Records Administration (NARA), National Personnel Records Center (NPRC), at St Louis, MO. The Offeror must give complete price for the purchase and delivery of the Archival Bond Paper. Please quote your price per ream and list shipping as a separate line item. Delivery is 2 shipments IAW the prescribed schedule. SCOPE: See the brand name Specification for Permalife Bond Paper (8-1/2" x 11" - 500 per ream - quantity 3,000 reams) and (8-1/2" x 14" - 500 per ream - quantity 1,000 reams). CLAUSES AND PROVISIONS: The following clauses and provisions are incorporated and are to remain in full force in any resultant fixed price purchase order: FAR Clause 52.212-1, Instructions to Offerors-Commercial Items (Feb 2012); The quotations will be evaluated in accordance with the procedures outlined in FAR Subpart 13.106-2 and award will be made to the offeror that submits the lowest price technically acceptable offer (factors considered: past performance, technical and price); each offeror shall include a complete copy of the provisions at FAR Clause 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2012), with the quotation. (1) Protests as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the General Accounting Office (GAO), shall be served on the Contracting Officer (address as follows) by obtaining written and dated acknowledgment of receipt from the: National Archives and Records Administration, BCN-Acquisition Division, 8601 Adelphi Road, Room 3340, College Park, MD 20740-6001, Attn: Kevin Schumacher. The copy of any protest shall be received in the office designated above within one day of filing a protest with GAO. Questions no later than December 21, 2012, by 4:00 PM ET. Quotes are due no later than January 2, 2013 by 4:00 PM ET. Vendors submitting quotes directly to the contracting officer are advised they should seek confirmation of receipt. NARA is not responsible for mis-routed (caught in spam filter) quotes. Quotation Requirements. Failure to comply with these requirements will render the vendor's quotation as incomplete and eliminate it from further consideration. Quotations must include the solicitation number, payment terms, identification of any special commercial terms, and must be signed by an authorized company representative. It is critical that Offerors provide adequate detail to allow evaluation of their quotes (See FAR 52.212-1(b)). Quotations shall be a minimum of 1 page and include specification for material used specifically for pH, calcium carbonate buffer percentage, Lignin and sulfur content, product cotton percentage, cellulose content, bond weight, acidity, color and delivery (location and quantity). If any doubt as to chemical composition of paper, vendor shall provide independent lab certification as to chemical composition. This is a brand name only specification IAW FAR 6.302-1(c). A copy of the justification required by this section is posted with this synopsis/solicitation. Past Performance will be evaluated as part of the technical evaluation. Vendors shall submit 3 references for similar work (See FAR 52.212-1(b) (10). Offerors will be responsible for downloading their own copy of this combined synopsis/solicitation and amendments (if any).
