Specific Tasks/ Education & Experience Requirements HYPERBARIC TECHNICIAN 1. QUALIFICATIONS 1.1 The technician will be a graduate of a (40) hour Undersea and Hyperbaric Medical Society (UHMS) Designated Introductory Course in Hyperbaric Medicine or the USAF Clinical Hyperbaric Technician Course within the first 6 months of hire. They shall be knowledgeable about compression physics and the mechanical effects of pressure change as it relates to the hyperbaric chamber and its occupants. The technician shall understand the physiological basis and clinical aspects of decompression sickness and arterial gas embolism. The technician will understand and use of U.S. Navy decompression tables DOBENDEM decompression tables, and hyperbaric patient treatment tables as directed. The technician shall set up and operate specialized medical equipment. Additionally, the technician shall apply chamber safety to the everyday working environment and follow emergency procedure checklists in the event of an emergency occurs. 1.2. Clinical Hyperbaric Technician (CHT), certification from the National board of Diving and Hyperbaric Medical Technology is desirable. CHT Certification requires 40 hours UHMS introductory course and 480 hours of clinical experience training in transcutaneous oxygen measurement and a certifying examination. The vendor shall be responsible for ensuring hired contracted employees complete the UHMS introductory course. The government will allow for the contracted employee to attain the 480 hours of clinical experience as part of their duty time at DGMC. The vendor shall ensure the technicians acquire CHT certifications within six months of hire date. Following certification, 12 hours of continuing education are required biannually. The technician will maintain CHT certification. Will accept a Licensed Vocational Nurse (LVN) with a minimum of 24 months clinical experience with in the past 36 months. 1.3. Certification in Basic Cardiac Life Support (BLS). Certification must be obtained through the American Heart Association and/or Red Cross approved training program. 2. SPECIFIC TASKS: The duties include but are not limited to the following: 2.1 Hyperbaric chamber duties: Performs hyperbaric chamber crew chief, chamber operator and inside attendant duties. 2.2 Have a working knowledge of and be able to trouble short issues with the atmosphere conditions system, gas farm, chamber lighting system, nitrogen services, patient monitoring system (cameras), portable water and chamber toilet, chamber oxygen monitoring, patient entertainment system, pressurization and depressurizations systems, fire suppression system chamber gas analysis system, mask breathing system, and automatic chamber control system. 2.3. Build, inspect, and maintain hyperbaric div masks. 2.4. Develop working of emergency procedures. 2.5. Prepare documents, turns in and tracks equipment in need of repair. 2.6. Prepare, issue and maintain file copy of temporary hand receipts for borrowed equipment. 2.7. Prepare and maintain hyperbaric diver files 2.8. Conduct training for hyperbaric divers 2.9. Conduct training for chamber crew positions (Chamber Operator, Crew Chief, and Inside Attendant). 2.10. Chamber operator duties include start-up, shut-down, and running the chamber. Chamber operator monitors patient’s conditions during hyperbaric treatment dive and coordinates care with the inside attendant and hyperbaric physician. 2.11. Crew Chief assists the chamber operator during routine operations, and also assists the chamber operator in the event of problems. Troubleshoots and corrects problems during the dive and between dives as necessary. 2.12. The inside attendant must meet and maintain standards for hyperbaric chamber duty IAW AFI 48-123, 6.48.5, as determined by a USAF flight surgeon. Must not be in a “duties not involving diving” status for longer than 90 days. The inside attendant will be certified with a minimum of three supervised training hyperbaric dives. 2.13. The technician should be expected to act as inside attendant at least once per week, depending upon patient chambers. 2.14. Inside attendant is inside the chamber during the treatment dive and monitors patients throughout the dive. The attendant will assist patients with equalizing pressure in their ears during the pressurization and depressurization. The attendant will assist with loading, unloading and positioning of patients in the chamber. 2.15. Attaches wound vacuum during dive, monitors glucose if necessary. 2.16. With few exceptions, prepares the procedure room/equipment in an efficient and timely manner, properly prepares and positions the patient and effectively assists providers I performing procedures. 2.17. Directly performs or assists in the performance of a number of specialized minor medical surgical procedures. 2.18. Prepares the procedure room and medical equipment for the procedure to be performed. Receives the patient and reviews his or her medical history. 2.19. Uses the nursing process and participates in making nursing assessments. Prepares and positions patient for the procedure and monitors vital signs. 2.20. Closely monitors the patient’s condition throughout the procedure and reports and//or notes changes in patient’s vital signs, symptoms, and general physical and emotional condition for inclusion in the patient care plan. 2.21. Performs procedures such as electrocardiograms (EKGs), urinary catheterizations, and suctioning of trachea. 2.22. Assists in performing exams, procedures and treatments such as gastrointestinal, respiratory and ENT. 2.23. Assists the physician in minor procedures. 2.24. Performs procedures such as would care, dressing changes, visual acuity tests, tonometry, EKGs, irrigation of eyes and ears, etc., in accordance with established procedures. 2.25. Documents the procedure in the patient’s chart. 2.26. Monitors and evaluates patient response during exams, procedures and treatments. 2.27. Collects and labels urine, sputum, blood and stool specimens. 2.28. Obtains nasal, through and other cultures. Inserts and drains in-dwelling catheters, removes sutures, changes dressings, irrigates wounds and performs venipunctures. 2.29. Adheres to established aseptic procedures to protect self, other staff members and patients from unnecessary exposure to infectious agents. 2.30. Cleans and prepares equipment for sterilization. 2.31. Disposes of infectious and hazardous waste materials in accordance with established procedure. 2.32. Assists in patient treatment process and disposition. Coordinates with medical and maintenance personnel on chamber equipment configuration and modification and patient hyperbaric treatment and hyperbaric research. 2.33. Performs inside attendant duties in hyperbaric chamber (dry land) oxygen treatment dives, assists patient IAW policies, regulations operating procedures and instructions. 2.34. Duties as required after accomplishing three training dives and any other appropriate training. 2.35. Strictly adheres to established aseptic procedures during medical and surgical procedures. 2.36. Complies with established procedures for disposing and infectious and hazardous waste materials. Administers prescribed medications. 2.37. Calculates dosages and administers medications by oral, topical, intradermal, subcutaneous, and intramuscular routes. 2.38. Initiates intravenous fluids to include heparin locks and monitors, regulates, and discontinues their usage. 2.39. Observes and promptly notifies nurse and or physician of adverse physical and emotional reactions to the medications. Exercises control over drugs. 2.40. Documents administration of medications in accordance with established charting procedures. 2.41. Recognizes the nature of emergencies and provides first aid in accordance with emergency protocols. 2.42. Initiates and maintains cardiopulmonary resuscitation (CPR) measures if first on the scene until additional responders arrive. 2.43. Furnishes physician and nurses with requested supplies. 2.44. Monitors the patient’s heart and respiration rate. 2.45. Conducts patient and family teaching. Teaches patients and family members how patients should perform simple tests for themselves, e.g., blood sugar, hypertension or urine. Informs patients and family members about self-care. Reinforces and reiterates instruction previously presented by the physician or nurse. Provides input for patient care planning. TRANSFER CENTER REGISTERED ER NURSE 1. QUALIFICATIONS. 1.1. Graduate from a nationally accredited associate or baccalaureate degree program in nursing and hold current certification as a registered nurse (RN). 1.2. Must have a least 24 months experience with the last 48 months as an ER nurse or working in a transfer center service position. 1.3. Certification in Basic Cardiac Life Support (BLS), Advanced Cardiac Life Support (ACLS), Certifications must be obtained through the American Heart Association and/or Red Cross approved training program. 2. SPECIFIC TASKS: The duties include but are not limited to the following: 2.1. Provides transfer center services for inpatient and outpatient care settings. Coordinates transfer process and network facilities to assist service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care. Demonstrate and incorporate principles of continuous process improvement and performance benchmarks to assist in managing and improving the Transfer Center Program. 2.2. Accepts all calls from external facilities 2.3. Ensures efficient and smooth processing of transfers. 2.4. Checks eligibility by verifying patient is registered in CHCS and DEERs. Updates demographic information as needed. Obtains current/accurate contact information. Maintains familiarity with any applicable Veteran’s Administrations Systems or contacts needed to verify eligibility. 2.5. Documents all medical information given by external facility appropriately. 2.6. Provides nursing consultant on specialty/surgical care needed in accordance with patient’s diagnosis. Ensures clarity in entry point for patient (ED or appropriate ward). 2.7. Remains continually knowledgeable of MTF capabilities and capacity to expedite outcome of transfer and accepting specialty/ward. 2.8. Facilities quick coordination with appropriate charge nurse in ward or nursing supervisor to verify bed availability for every transfer that is within capability. 2.9. Facilitates appropriate doctor-to-doctor calls. Ensures sending facility is aware the doctor must call to give current report. 3.0. Coordinates with sending facilities to ensure all appropriate medical records are available for review upon patient arrival to facility. 3.1. If during off-peak transfer hours, may be able to assist ED clinical team with all patients being accepted to the ED. Remain current on clinical skills to expedite patient care and to assist ED nursing staff on the floor with all transfer patients. Transfer Center phone must still be answered. 3.2. Establishes working relationship with Admissions staff to ensure appropriate admissions occur in a timely fashion. 3.3. Shall participate in activities in activities to the extent required by the MTF QA plan or regulation. The government will evaluate the RNs professional performance under this contract using quality assurance standards specified in AFI 63.14, Medical Quality Operations. 3.4. Collaborates with MTF departments for program planning, development and evaluation in support of the MTF Strategic Plan/Mission Support Plan. Develops interdisciplinary management style through effective communication and collaboration with appropriate departments. 3.5. Delegates tasks to appropriate level and directs support staff in meeting MTF operational requirements. 3.6. Facilitates process improvement to enhance the delivery of nursing care. 3.7. Conducts inspections as directed by requesting office/regulatory body. 3.8. Provides for continued professional growth of assigned staff. 3.9. Leadership and/or participation in committee and/or functions associated with the facility that might include but are not limited to; Nursing Practice, Infection Control, Education and Training and similar. 3.10. Participates in professional development activities and maintains professional affiliations. 3.11. Attends conferences and workshops related to duties. 3.12. Maintains current knowledge of disease management, preventive screening standards/requirements, and current trends in population-based health management. 3.13. Maintains current knowledge of practice standards and facilities compliance. 3.14. Discusses, supports and advocates ethical decision making for staff and patients. 3.15. Champions change by challenging status quo; acts as catalyst when appropriate and manages implementation effectively. 3.16. Provides input to educate caregivers and patients on prevention/disease management programs. 3.17. Effectively utilizes information systems to retrieve, analyze, update, and record nursing data. 3.18. Completes reports to provide current and meaningful data from which assessments can be made to determine effectiveness of prevention efforts. 3.19. Meets all suspense dates 3.20. Prioritizes requirements and makes timely and appropriate decisions impacting nursing care. 3.21. Accesses information systems to analyze, track and quantify program effectiveness, and to manage the dissemination of information critical to the success of the population health management. 3.22. Forwards questions regarding medical necessity, appropriateness of care, and quality concerns to PCM providers sing established guidelines. 3.23. Intervenes with appropriate individuals/departments regarding delays in service that may impact quality patient care. 3.24. Demonstrates strong customer service skills and fosters development of those skills throughout entire MTF. 3.25. Communicates and collaborates with the network facilities and appropriate members of the health care team to ensure that needs of assigned patients are met without duplication of efforts. 3.26. Coordinates and participates in interdisciplinary team meetings, designated facility meetings and Care Coordination meetings. Shares knowledge and experiences gained fro own clinical nursing practice and education relevant to nursing. 3.27. Participates in the orientation and training of other staff. May serve on committees, work groups, and task forces. 3.28. Must maintain a level of productivity and quality consistent with complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional nursing. 3.29. Completes medical record documentation and coding and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies and guidelines. 3.30. Completes all required electronic medical record training, MTF-specific orientation and training programs and AF/DoD mandated nurse specific training.
