- SOW This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will NOT be issued. Solicitation Number HSCG84-13-Q-PD4046 is issued as a Request for Quote (RFQ). The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-64, and may be obtained electronically at http://www.arnet.gov/far. This RFQ is issued as unrestricted, and offers are solicited from all responsible business concerns. The applicable NAICS code is 811219, and the small business size standard is $19.0M. Offers are due at the U. S. Coast Guard SILC (PCB31), Attn: Contracting Officer, Suite 950, 300 East Main Street, Norfolk, VA 23510, by 3:00 p.m. EST on February 28, 2013. DESCRIPTION OF REQUIREMENT: Provide annual calibration, repair, and testing of PortaCount Pro Respirator Fit Tester, Model 8030, 26 each, per attached statement of work (SOW). The period of performance is for a base year (12 months from award date) and four 1-year options. Offers/Quotes must include: (1) The solicitation number; (2) The name, address, and telephone number of the offeror; (3) Unit pricing; (4) Company Tax Information Number and DUNS Number; (5) References for past performance in calibration, repair, and testing of equal or similar respirator fit testers; and (6) If the offer is not submitted on Form SF-1449 include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation.
