- The Ohio Army National Guard (OHARNG) is looking for a vendor who can provide a year maintenance service agreement, to include calibration for a commercially off the shelf Computer Screening, Inc. (CSI) model 3000Z free standing Blood Pressure (B/P) kiosks. Request a firm fixed price for the service items identified below for 12 B/P Kiosks located through various locations in the state. Key elements of the service agreement include: a.Onsite service response within 72 hours as initiated by the customer. b.Adjustments and replacement of parts as necessary for the machine to function. If the tech cannot repair an item, it will be replaced. The contractor will furnish maintenance parts on an exchange basis. Parts and on-site labor (travel included) are provided at no additional cost. c.Service availability during Monday - Friday is from 0900 to 1700, except Holidays. This service is to keep the blood pressure kiosk(s) in good working order or to restore the health station kiosk(s) to good working order based upon the specific needs of the individual blood pressure kiosk(s) as determined by the manufacturer. d.Preventive maintenance on OHARNG blood pressure kiosks may be provided at locations near routine or on-call visits made by the technicians across Ohio on a case-by-case basis. Ideally coordinating such visits with OHARNG sites will allow for adequate access to OHARNG facilities. Otherwise, service to OHARNG blood pressure kiosks will be performed at an on-call basis. The OHARNG will contact the contractor for service as needed and identify two (2) Primary POCs for facility/site scheduled visits. Following the initial call, the contractor will schedule a technician visit coordinating date and time with the State POCs and provide the technician with office and cell phone number of the 2 POCs. At the end of the year maintenance, the vendor must notify Government 90 days prior to expiration. WARRANTY & MAINTENANCE GUIDELINES Full twelve-month warranty for parts & labor. Field Service Technicians are defined as trained field technicians or independent contractors to install and maintain kiosks on a state scale. Service Representatives shall respond to all service calls within 72 hours. TECHNICAL SUPPORT Technical support may be provided over the phone or via email. Resolution within 24 hours excluding Saturday, Sunday and Holidays. Additional resolution time of 1-3 business days may be required for issues requiring additional information or requires a system adjustment.
