QUESTION and ANSWERS to VA243-13-R-0389 Can you tell me what page the requirements for the cover sheet is on - I have read the solicitation cover to cover and did not see it Answer The proposal submission must consist of two separate volumes entitled: "Volume I: Price Proposal" and "Volume II: Technical Proposal". The Contractor shall furnish an original hard copy, five (5) hard copies and an electronic copy of both Volumes I and II to linda.williams11@va.gov. The Contractor's proposal must consist of two separate binders and two documents in email for Volume I and Volume II individually titled and numbered for manuals both on exterior of the top covers. The Contractor's package must be labeled with the Contractor's name and business address of firm, and the solicitation number Part 3: Price Calculation Method leading to PER Diem costs (including labor categories, expected hours worked, hourly rates, and any discounts to perform the tasks in the SOW, to include any proposed transportation costs that may be proposed). I understand the location of the beds - but where do you want this information placed in the price worksheet Answer On the Volume I Price Proposal Requirements: 1.Other cost: can you explain what needs to be in this section 2.What needs to be on the cover sheet 3.It's states that we need to put in location - where? Answer VOLUME I: PRICE PROPOSAL REQUIREMENTS. The Contractor shall submit a Fixed Price Proposal for direct, indirect, and any other associated costs proposed for this effort, including the cost of any proposed subcontractors/consultants. The Price Proposal must include three parts: Part 1: Cover Sheet. Part 2: Completed Pricing Sheet (see PRICE WORKSHEET) with Locations and proposed amount of availability use a separate sheet. Part 3: Price Calculation Method leading to PER Diem costs (including labor categories, expected hours worked, hourly rates, and any discounts to perform the tasks in the SOW, to include any proposed transportation costs that may be proposed). Several questions: 1. Do all of the copies need to be in binders or only the original Price/Technical Volumes proposals? 2.Is it permissible in the eyes of the VA for this solicitations purpose to have 2 residents to a room? 3.As the deadline is March 17, 2013 which is a Sunday, is it ok if the package is post marked by this date, or does it have to be in the Bronx by the 17th? 4.On the Pricing sheet, what does the term "Other Cost" refer to in the first column? Answer 1.That will be fine 2.Yes 3.Be in the Bronx by 17th 4.Note any other items but not building, furniture costs ets. Things like travel, meals etc need to be separately noted per veteran Are there travel expenses to conferences/meetings related to this solicitation? Or are we just looking at travel for our staff to the project location? Answer Travel for the veterans Is there anything else besides travel that should be in that line? Like one-time expenses? ANSWER No one time expenses. The per-diem rate will remain the same for the year I did not see the word AMENDMENT on the solicitation. I want to be sure that I am reading the correct RFP. Kindly send me a link to the most recent version of the RFP. I have a couple of questions: 1.Is there a maximum per diem amount that can be requested? 2.If there is no maximum per diem, is there a maximum total amount that can be requested? 3.When will the solicitation/RFP come out for the period October 1, 2013 to September 30, 2014? 4.Will new residential treatment programs be invited to apply at that time? Answers Please enter what rate you access for the services There is no solicitation to come out for that period. Just from award of this one till Sept 30, 2013 with 4 one year options End
