Appendix C Appendix A Performance Work Statement Request for quote. Combined Synopsis/Solicitation (IAW FAR 12.603) This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Air Force Sustainment Center (AFSC/PZIMA) at Tinker Air Force Base Oklahoma, intends to negotiate a firm fixed price contract with a period of performance of 19 April 2013 - 19 June 2013 for the one time test, teardown, evaluate, and repair of the LVD Strippit Punch Press requirement. This solicitation is issued as a request for quotation (RFQ) FA8125-13-Q-0027. Contractor is required to sign this document and return 1 copy to issuing office. Contractor agrees to furnish and deliver all items and provide all services set forth or otherwise identified above and on any additional sheets subject to the terms and conditions specified herein. Proposal Submission Information: Proposals are due by 29 March 2013, 4:00 PM, Tinker AFB OK Central Standard Time (CST). Ryan Contracting Officer Melissa.ryan@tinker.af.mil Phone: (405)-739-5799 EVALUATION OF RESPONSES: Award will be conducted and evaluated under the provisions of FAR Part 12, Commercial Items, FAR Part 13, Simplified Acquisition Procedures. The responsible offerors must be registered in the System for Award Management (SAM) database (available at: https://www.sam.gov/portal/public/SAM) system per FAR 52.212-3, and submit a completed copy of the Offeror Representations and Certifications - Commercial Items with their proposal. Offerors must also be registered in the Central Contractor Registration database (available at: www.ccr.gov) per FAR 52.212-1. Lack of registration in SAM will qualify contractor as ineligible for award. All responsible sources may submit a proposal in response to this solicitation, which shall be considered. This solicitation is being issued as a Request for Quotation (RFQ), for a firm fixed type commercial contract. The Air Force Sustainment Center (AFSC/PZIMA) at Tinker Air Force Base Oklahoma intends to issue a sole source contract for the two (2) month T,T&E, and repair of the LVD Strippit Punch Press requirement. Clauses 52.212-1 and 52.212-2 provide instructions for submitting proposals and the evaluation criteria to be used in the evaluation of offers. Please note Clause 52.212-4(l) Termination for the Government's convenience has been modified within this solicitation.
