Description
SOW/Tech.Eval. This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included in this notice. THIS ANNOUNCMENTS CONSTITUTES THE ONLY SOLICITATION; A SEPARTE WRITTEN SOLICITIATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Center for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a fixed priced purchase order at the a fixed labor rate for Financial Data in accordance with the attached Statement of Work. It is anticipated that the period of performance will be 12 months. The North American Industry Code (NAICS) is 523999. The Request for Quotation is being set aside for small business concerns. Quotes should reflect one base year and two option years. Responses must be sent electronically in MS Word format to renee.wallaceabney@cms.hhs.gov no later than 2:00 p.m. EST, April 15, 2013. Responses shall be no more than fifteen pages and shall include the following: 1) Contractor's Plan to perform the Statement of Work; 2) Fully loaded labor rates and anticipated hours. All responses will be evaluated by utilizing the Lowest Price Technically Acceptable (LPTA) Source Selection Process. The lowest price technically acceptable selection process is appropriate when best value is expected to result from selection of the technically acceptable process; for one base year and two optional years. All responses shall be submitted in whole dollar amounts. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies conflict, the vendor shall be responsible for providing mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may be terminated.
