- STATEMENT OF WORK Background: A Mock Joint Commission Survey shall be conducted at each Veterans Integrated Service Network 18 (VISN) facility to assist in the continuous readiness for unannounced Joint Commission Surveys. The Mock Survey process shall focus on the National Patient Safety Goals, frequently cited requirements for improvement, survey process, human resource readiness, and tracer methodology. The Contractor shall possess knowledge of Joint Commission (JC) standards and VHA regulations which apply to accreditation bodies. The Survey process shall identify important organizational and systems issues related to safety, quality of care, treatment and services and interfaces between such processes. An educational session shall be provided for each VISN facility focusing on, but not limited to: the National Patient Safety Goals, frequently cited requirements for improvement, survey process, and human resource readiness. The educational session date and content shall be negotiated between the facility and the consulting firm. Specific Requirements: 1.Conduct a Mock Joint Commission Survey at each of the VISN 18 facilities. Each survey shall last three days per facility with the exception of two days for El Paso and Big Spring. Each consultant team for the three day surveys shall have four members consisting of a physician consultant surveyor; nurse consultant surveyor; a nurse administrative consultant surveyor; and a life safety engineer. Each survey team for the Two day surveys will include a physician consultant surveyor; nurse consultant surveyor and a life safety engineer. Each consultant shall have a minimum of three years of surveying experience. Each of the consultants shall have experience, education, and knowledge of infection control processes. 2.At the Albuquerque, New Mexico facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Domiciliary, Psychiatric Residential Rehabilitation Treatment, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Review of the Community Based Outpatient Clinics. Address: 1501 San Pedro SE, Albuquerque NM 87108. 3.At the Amarillo, Texas facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Substance Abuse Residential Rehabilitation Treatment, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 6010 Amarillo Blvd West, Amarillo TX 79106. 4.At the Big Spring, Texas (West Texas) facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Domiciliary, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 300 Veterans Blvd, Big Spring TX 79720. 5.At the El Paso, Texas facility, services shall include a review of the Outpatient, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 5001 N. Piedras, El Paso TX 79930. 6.At the Phoenix, Arizona facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Substance Abuse Residential Rehabilitation Treatment, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 650 E. Indian School Road, Phoenix AZ 85012. 7.At the Prescott, Arizona facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Residential Rehabilitation Treatment, Domiciliary, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 500 Hwy 89N, Prescott AZ 86313. 8.At the Tucson, Arizona facility, services shall include a review of the Hospital, Community Living Center, Outpatient, Substance Abuse Residential Rehabilitation Treatment, Mental Health Intensive Case Management, Home Care, and Community Based Outpatient Clinic programs. This review will be a three-day review. Address: 3601 S. 6th Ave, Tucson AZ. 9.Dates of the surveys to be negotiated with each facility. However, all onsite surveys shall be completed no later than August 31, 2013. After each facility survey is completed, the surveyor shall schedule an exit conference with the Facility Director and other leadership staff to discuss findings and trends. 10.Consultants shall use the Joint Commission Standards published in the Comprehensive Accreditation Manual for Hospitals: The Official Handbook (CAMH) and other appropriate standards for Long Term Care, Ambulatory Care, Behavioral Health, and Homecare. 11.Each facility shall supply the surveyor with accurate information describing the scope of services provided. The facility shall provide access to those staff members who shall normally be involved in Joint Commission Survey to allow adequate assessment of standards compliance. 12.The hospital surveys shall be coordinated with the local Quality Management or Accreditation Staff at each site. All of the surveys shall be completed by August 31, 2013. Each survey shall include an opening conference; patient tracer activities, leadership conference, exit briefing and standards based report. 13.The consultants shall conduct a facility-specific educational session at each VISN facility focusing on, but not limited to: the National Patient Safety Goals, frequently cited requirements for improvement, survey process, and human resource readiness. Each educational session shall accommodate approximately 40 VA Employees; the VA shall provide a facility to conduct the session. The educational session date and content shall be negotiated between the facility and the consulting firm. Experience in developing on-going educational programs both at The Joint Commission and at the collegiate level required. 14.The consultants shall possess the appropriate qualifications and credentials for conducting the surveys. They shall have surveyor and consultant experience and be knowledgeable of the unique challenges faced by VA Medical Centers. The surveyors shall have in-depth knowledge of VA's Computerized Patient Record System (CPRS) and history of data security compliance with VA medical records. Additionally, they shall have extensive VA Medical Center consulting experience including SARTPs, PTSD, Polytrauma, and psychosocial rehabilitation programs in addition to hospital, behavioral health and long term care programs. VISN 18 facility knowledge preferred, military experience preferred. Must guarantee consistent team membership unless for an approved emergency. This requirement must be completed by 08/31/2013. 14.Consultants must possess track record of experience assisting Medical Centers with successfully demonstrating evidence of standards compliance after Joint Commission survey findings. Previous Joint Commission consultation with no adverse decisions, resulting in full accreditation award(s). 15.Four member team comprised of a combination of medical doctor, registered nurse, and a registered nurse with combined experience in Hospital, Home Care, Ambulatory, Behavioral Health, and Long Term Care Joint Commission Standards, and a Life Safety Specialist.. Members must possess previous experience as Joint Commission certified surveyors. The surveyors shall have expertise in Infection Control Practices. 16.Team members shall have experience with Special Survey Unit (SSU) at the Joint Commission - preferably conducting for-cause announced and unannounced surveys and preparing root cause analyses. 17.All costs, including travel, lodging and materials furnished to the facilities/VISN must be included in the quote submitted. Payments shall be made monthly, based on the successful completion and acceptance of onsite work for each medical center.
