Description

Notice of Proposed Contract Actions IAW FAR 12 and 13, RFQ VA-259-13-R-0083 The Sheridan Veterans Affairs Medical Center (SVAMC), Sheridan, WY and VISN 19 / Network Contracting Office (NCO) 19 Rocky Mountain Acquisition Center (RMAC) contemplate an award of a Total Small Business Status, Firm Fixed Price Contract for Podiatry Services. The proposed contract will be issued for commercial firm fixed price contract types. The period of performance for these contracts will be one year (base year), with four additional, one-year option periods from date of award, which is estimated for July 01, 2013. While price will be a factor in the evaluation of offers, the final contract award shall be based on best value which includes contract qualifications, past performance, quality assurance, and regulatory practices. Request for Proposal VA-259-13-R-0083 will be available for download on or about noon April 12, 2013. No telephone requests for this solicitation shall be allowed. Offers (fully completed proposals) are to be received at the VA NCO 19, RMAC, 4100 E. Mississippi Ave., Suite 900, Glendale, CO 80246 and/ or electronically edwar.marshburn@va.gov, no later than May 09, 2013, 4:00 PM Mountain Standard Time. The NAICS Code is 621391 with a Small Business Size Standard of $ 7 Million. Additional Requirements: Contractors shall be required to meet the standards of the Joint Commission and to provide current Podiatry licensure and proof of insurance.
