- The Department of Veterans Affairs, Orlando VA Medical Center (OVAMC), Orlando FL, intends to solicit and negotiate with only one source under the authority of FAR 6.302-1 (a)(2); Only One responsible Source Available, no other source will satisfy agency requirements. The VA intends to solicit and negotiate with A - Ability Medical Equipment, Inc., 6765 Narcoossee Road, Orlando, FL 32822. This notice of intent is not a request for competitive proposals. However, any responsible source who believes it is capable of meeting the requirement may submit a capability statement, proposal or quotation, which shall be considered by the agency, only if it demonstrates that it is qualified, capable and meets all qualifications stipulated in the SOW below. To be considered proposal shall be received by the closing date and time of this notice. A determination not to compete the proposed requirement based upon the responses to this notice is solely within the discretion of the government. Responses to this notice are due on or before April 19, 2013, 0800 EST by email to Alonzo.jacobs@va.gov. Point of contact for this notice is Alonzo Jacobs, at Alonzo.jacobs@va.gov. Contractor shall be responsible for provision of all labor, a secure warehouse facility, tools, transportation, dispatch services and management for delivery and pick-up of Durable Medical Equipment (DME) to VA beneficiaries of the OVAMC and the following affiliated VA medical facilities: Viera Outpatient Clinic (OPC), 2900 Veterans Way, Viera, FL 32940 and Daytona Beach OPC, 551 National Healthcare Drive, Daytona Beach, FL 32114. The service shall include all labor, tools, transportation, secure warehouse facility, dispatch services and management for delivery and pick-up, receiving, storage, set-up, assembly, take-down, disassembly, minor installation, repair and maintenance of Government Furnished Durable Medical Equipment as required by the OVAMC and affiliated OPCs. The contractor shall also conduct DME safety inspections and educate VA beneficiaries or VA beneficiary care providers on the proper use and functions of DME. The contractor shall have a secure warehouse within a twenty-five (25) mile radius of the OVAMC that houses the contractor's vehicle dispatch service, secure storage of Government Furnished Equipment (GFE), repair and maintenance operations and related functions for the duration of the contract. The contractor shall be fully accredited by the Joint Commission or meet Joint Commission standards in the performance of work for the entire term of this contract. Contractor shall be in compliance with Health Insurance Portability and Accountability Act (HIPAA) and shall have a minimum of three (3) years' experience in vehicle dispatch service, storage, delivery, pick-up, handling, set-up (assembly and disassembly) and sanitization of DME, educating patients on proper use and function of DME that includes providing patients instructions on proper emergency measures/steps/procedures in the case of failure or imminent failure of DME.
