Description
The Department of Veterans Affairs intends to negotiate a noncompetitive award to Veteran Artist Program to prepare and complete the tasks associated with organizing and producing workshops and public performances for the San Francisco VA Medical Center. This procurement is a sole source award under the Veterans Benefits Act of 2003 in accordance with FAR 13.106-1(b)(1) Soliciting from a single source. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Sole source justification has been prepared in support of this acquisition. This notice is NOT a request for competitive proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-56. The NAICS code is 711320 and the small business size standard is $7.0 million. STATEMENT OF WORK San Francisco VA Medical Center Community Integration Through Creativity III Project 1.0 General Information: Contractor shall provide preparation for and complete all the Community Integration Through Creativity III (CITC3) tasks (see Deliverable Tasks) associated with the workshops, public performances and sustainability requirements outlined by the San Francisco VA Medical Center (SFVAMC). 2.0 Performance Period: The period of performance is from June 1, 2013 through August 31, 2013. 3.0 Contract Costs: Contract costs shall include salaries, fringe benefits, equipment and venue costs, and liability insurance expenses. 4.0 Contractor Personnel Qualifications: Contractor personnel shall perform services in accordance with ethical, professional, and technical standards of the academic healthcare industry. Contractor Personnel shall be technically proficient in the skills necessary to fulfill the requirements stated herein including the ability to understand, speak, read and write the English language. 5.0 Contractor Personnel Duties and Scope of Work: 5.1 Deliverable Tasks 5.1.1.Task 1 - Community Workshops 5.1.1.1.Secure workshop venues 5.1.1.2.Secure workshop leaders 5.1.1.3.Secure workshop equipment 5.1.2.Task 2 - Public Performances/Speakers 5.1.2.1.Conduct outreach and marketing 5.1.2.2.Secure professional artists, speakers and mentors 5.1.2.3.Purchase performance supplies and equipment 5.1.3.Task 3 - Public Events Rental/Summit Meetings 5.1.3.1.Secure event/venue rentals and equipment 5.1.3.2.Secure meeting locations 5.1.4.Task 4 - Exhibition Contractors, SF and CBOCs 5.1.4.1.Purchase equipment for CITC3 event 5.1.4.2.Secure labor to support CITC3 event 5.1.5.Task 5 - VA Training Manuals and Promotional Materials 5.1.5.1.Conduct data collection and surveys 5.1.5.2.Purchase materials and provide training 5.1.6.Task 6 - PSAs, Videos, Stills and Documentary 5.1.6.1.Conduct interviews of workshop attendees 5.1.6.2.Purchase necessary equipment and gear 5.1.6.3.Take still pictures and videos of all events 5.1.6.4.Shoot public service announcements 5.1.6.5.Provide written training manuals for event replication 5.2. Contractor personnel shall be under the overall direction of the Director, Psychosocial Rehabilitation and Recovery Center. 5.3. Contractor personnel shall by virtue of training and experience be able to perform all levels of training and instruction related services on pre-vocational and vocational skills related to specific creative arts and arts exhibitions "Providing both training and practice on production and marketing of art for the CITC3 Initiatives "The full program will take multiple sessions, in-person; "The schedule will be flexible and adjusted in accordance with the needs of the participants. "Contractor will train PRRC clients in pre-vocational skills related to specific creative arts skills and exhibitions of art work, as well as implement and monitor evaluation survey of exhibitions. "Contractor will act as liaison with community and other VA partners to promote the Veterans' arts workshops, exhibitions, public performances and community events. "Contractor will travel as needed to VA Community-Based Outpatient Clinics and other community-based sites to assist with outreach activities. Expenses will be reimbursed in accordance with standard VA policy. 6.0 Personnel Policy, Hours, Monitoring and Payment: 6.1. The parties agree that Contractor personnel working under this contract will not be considered Government employees. 6.2. The Government shall maintain a record keeping system for verification of procedures performed. Contractor personnel shall follow all established timekeeping procedures used to audit and approve Contractors invoices for payment. Contractor will bill and be paid for actual services provided by Contractor personnel. 6.3 Payments will be processed upon receipt of invoices on a monthly basis for services already completed. 6.4 The Contractor shall provide the necessary licenses/certificates, competencies, privilege and credentialing in accordance with applicable State(s) and Federal regulation for each employee that will perform services under this contract. 7.0 Government Furnished Property: The Government may furnish equipment and supplies required for performance under this contract. Contract personnel shall be responsible for requesting necessary supplies and be responsible for routine maintenance of Government furnished equipment. 8.0 Licenses, Certifications and Insurance: All work related to this contract will be performed by the Contractor in accordance with any applicable OSHA regulations, as well as applicable State health and safety regulations. 9.0 Designation of Contracting Officer's Representative (COR) 9.1 The Contracting Officer (CO) will appoint a Contracting Officer Representative (COR) to perform tasks related to the execution and surveillance of this contract. The COR will be designated to represent the Contracting Officer (CO) in furnishing technical guidance and advice under this contract. The foregoing is not be construed as authorization to interpret or furnish advice and information to Contractor relative to the financial or legal aspect of the contract. Those matters are the responsibility of the CO and shall not be delegated. 9.2 The CO is responsible for the administration of this contract. The CO is the only individual authorized, to extent indicated in this contract, to take actions on behalf of the Government, which may result in changes in the contract terms, to include deviation from the statement of work, details, and performance schedules. Communications pertaining to contractual matters shall be addressed to the CO. No changes in or deviation from the scope of work shall be affected without a written modification to the contract executed by the CO. No oral statements of any person whosoever will in any matter or degree, modify or otherwise affect the terms of this contract. 9.3 The Contractor will receive a copy of Delegation Letter which will identify the COR appointed by the CO. 10.0 Modifications: 22.1 The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The contractor shall communicate with the Contracting Officer in all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes, which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustments will be made in the contract price to cover any increase in costs incurred as a result thereof.
