- Department of Veterans Affairs Veterans Health Administration Office of Informatics and Analytics June 14, 2013 VA701-13-I-0242 REQUEST FOR INFORMATION - SOURCES SOUGHT NOTICE VA Terminology Standards Advancement INTRODUCTION: This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced, and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. PURPOSE: Support to the Veterans Health Administration (VHA), Department of Veteran Affairs, to accelerate the pace of clinical terminology standards development and integration in areas that support Veterans health care and benefits determination. The VA would like to accelerate the enhancement of clinical terminology standards and related infrastructure for internal uses such as clinical decision support, quality improvement, research and business processes and for external data sharing with key partners including the Department of Defense and VA academic affiliates across the country. OBJECTIVES: 1. BACKGROUND: The Office of Informatics and Analytics, Health Informatics, Veterans Health Administration, Department of Veterans Affairs is entering into an Interagency Agreement (IAA) with the National Library of Medicine (NLM), National Institutes of Health (NIH), Department of Health and Human Services (HSS) for this purpose because NLM is the designated HHS central coordinating body for clinical terminology standards required for meaningful use of electronic health records (EHRs) in the United States. In this capacity, NLM develops and disseminates the RxNorm clinical drug terminology, provides substantial funding for the development and dissemination of Logical Observation Identifiers Names and Codes LOINC, and serves as the US Member of the International Health Terminology Standards Development Organisation (IHTSDO), thereby supporting US-wide use of Systematized Nomenclature of Medicine-Clinical Terms (SNOMED CT) and the creation of the US Extension to SNOMED CT. NLM has in place mechanisms, processes, and expertise that can be used to accelerate clinical terminology development and integration in VHA priority areas. 2. SCOPE OF WORK: The scope of the work to be accomplished under this IAA includes acceleration of the following: A.Modifications and additions to SNOMED CT and/or the US Extension to SNOMED CT, to LOINC, and to RxNorm so they evolve more rapidly and effectively to support current and emerging priority use cases for the VHA and its federal and private sector partners. B.Coordinated enhancements to the IHTSDO Workbench so that NLM and VHA development efforts are mutually beneficial and support more rapid improvements to SNOMED CT and more effective mapping and/or integration of SNOMED CT with other relevant health IT standards. C.Expansion of the capabilities of the NLM Value Set Authority Center as needed to support more effective authoring, validation, maintenance, and use of vocabulary value sets for clinical decision support and other high priority VHA use cases, as well as for clinical quality measures. 3. TASKS: The VHA and NLM have identified the following list of high priority tasks to be addressed under this IAA. In line with the VA's required agile system development methodology, agreed deadlines for early steps will be established, and resource allocation among the tasks will be adjusted, as knowledge of the requirements and complexity of each task increases and/or their relative priority for the VA changes. A.Establish principled relationships between LOINC and SNOMED CT so that they form an interlocking set to support effective integrated use by the VA and other US health care providers. 1.Finalize specific rules and parameters for relating and connecting LOINC and SNOMED CT content in the laboratory test domain and initiate analysis for other domains. 2.Determine the magnitude and prioritize the changes that must be made to LOINC and SNOMED CT to instantiate these relationships and connections in light of the VA's priority use cases. 3.Make changes to SNOMED CT and LOINC at the rate and volume commensurate with resources needed and available for this task. B.Modify and add content to SNOMED CT to support more accurate recording of functioning, disability, and health status and effective linking to the World Health Organization (WHO) International Classification of Functioning, Disability, and Health (ICF). 1.Review IHTSDO analysis of work required to align SNOMED CT and ICF (final report to be issued in August 2013) and identify the proposed changes to SNOMED CT most relevant to VA's priority use cases. 2.Make changes to SNOMED CT and LOINC at the rate and volume commensurate with resources needed and available for this task. C.Evaluate and possibly implement method for identifying, adding, and tagging "prescriber-friendly" medication names in RxNorm. 1.Test a potential method for identifying and adding "prescriber-friendly' medication names to RxNorm. 2.If the test is successful, develop and implement a production workflow for this process at the rate commensurate with resources needed and available for this task. D.Fully model Global Medical Device Nomenclature (GMDN) content that is incorporated in SNOMED CT 1.Identify categories of medical devices relevant to VA's priority use cases. 2.Fully model these high priority categories within SNOMED CT at the rate and volume commensurate with resources allocated to this task. E.Revise and expand the diet and nutrition content in the US extension to SNOMED CT to support high priority use cases. 1.Based on existing input and requests from the Academy of Nutrition and Dietetics, the S&I framework Health eDecisions group, and the NIH Clinical Center, produce a new hierarchy for nutrition and dietetics in the US Extension to SNOMED CT for later incorporation into the international release, with a focus on work relevant to the VA's priority use cases and at a rate commensurate with resources allocated to this task. F.Review VHA and NLM current implementations and capability priorities for the IHTSDO workbench and determine if additional work is needed in this area to support other high priority tasks under this IAA. G.Review current NLM Value Set Authority Center capabilities and development plans and determine if additional work is needed to support VA's high priority use cases. 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 15 pages in length, single-spaced, with a 12-point font minimum. The Government will not review any other data or attachments that are in excess of the 15 pages. Companies shall provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. A.Interested parties shall furnish the following information: 1.Company name and address 2.Company point of contact name, telephone number, and email address 3.Company business size and status (i.e. Large Business, Small Business, Service Disabled Veteran Owned Small business, Woman Owned Small Business, etc.), the number of years in business, and affiliate information. 4.If the company holds a Federal Supply Schedule (FSS) Contract list the GSA Contract Number and relevant SINS. 5.Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities, qualifications, and skills the company possesses to perform services described in the scope of the work. B.It is requested that the above information be provided no later than 11:00 a.m. Monday July 01, 2013. Responses should be emailed to Steve Mock, Steven.Mock@va.gov.
