Description
This shall be a Service Disabled Veteran Owned Small Business Set-Aside (SDVOSB). As a result, all SDVOSB's may submit a bid, which shall be considered by the agency. The project falls under NAICS Code 236220, and has a Small Business Standard of $33.5 Million. Project No.: 630-11-109 Project Title: Renovate Women's Health Clinic - 7 W SCOPE OF WORK Provide all labor, materials, equipment, Personal Protective Equipment (PPE), and supervision necessary to perform demolition and construction for 7 West of the Main Hospital Building for the Veteran's Affairs Medical Center located on 423 East, 23rd Street, New York, New York 10010. Renovation shall include new floors, walls, ceilings, lighting and reworking of HVAC, plumbing, electrical and sprinklers as required by drawings and specifications. The work areas shall be included but is not limited to: 1. PHASE 1: Medical Media Area ( ±1,520 SF) A. Remove selected furniture and equipment B. Demolish existing Medical Media area C. Construct new (8) exam rooms (#1 thru #8) including corridors D. New patient closet E. New clean utility room F. New soil utility room G. New house keeping room H. New storage room I. New electric closet J. New male toilet room K. New female toilet room 2. PHASE 2: Employee Education Room 7027W ( ±289 SF) and Main Corridor A. Remove furniture and equipment B. Construct new (1) male and (1) female lockers rooms C. Main Corridor: Remove 1 pair of door and hardware Remove 1 pair of fire-rated door, hardware and associated fire alarm Components Install 1 pair of fire-rated door, hardware and associated fire alarm Components and testing 3. PHASE 3: Staff Toilet and Lockers Rooms ( ±706 SF) A. Main Corridor: Remove 1 pair of door and hardware B. Female Toilet Room 7003W: Remove door and close wall C. Female Lockers Room 7004W: Remove lockers and construct new 16-patient waiting/reception room D. Male Staff Toilet Room 7006W: Remove door and close wall E. Male Lockers Room 7005W: Remove lockers and construct new Pain NP exam room (#9) 4. PHASE 4: Staff Offices ( ±1006 SF) A. Room 7033W: Close hall-door and add new door and hardware at west wall between Room 7032W and 7033W B. Room 7032W: Remove furniture and equipment, construct new (1) office and (1) office/exam room (no sink) C. Room 7030-W: Remove furniture and equipment, construct new exam room #10 D. Room 7029: Remove furniture and equipment, construct new exam room #11 E. Room 7028-AW: Remove furniture and equipment, construct new exam room #12 F. Room 7028-BW: Remove furniture and equipment, paint all interior walls to match existing G. Room 7026W: Remove furniture and equipment, paint all interior walls and ceiling to match existing H. Room 7024BW: Remove furniture and equipment, paint all interior walls and ceiling to match existing 5. PHASE 5: Main Corridor ( ±1090 SF) A. Replace ceiling tiles, T-grid, light fixtures, floor tiles and cove bases with new or approved equal. Paint corridor walls to match existing or approved equal. ASBESTOS ABATEMENT Contractor shall provide certified labor, materials, asbestos abatement and air monitoring as required by drawings and specifications and disposal of ACM in accordance with the New York State rules and regulations. WASTE DISPOSAL Contractor shall transport all waste and debris created during the project to the proper disposal areas staged outside of the facility. Contractor shall provide waste removal services for debris disposal, asbestos waste, medical waste, hazardous and nonhazardous chemical wastes, dumpsters, compactor garbage trucks and landfill fees, etc. GENERAL REQUIREMENTS 1. Contractor's employees must complete VA "privacy and information security awareness training". This is a mandatory training for all VA and non-VA employees working at this facility. Completion certification must be provided for each employee that will be onsite. 2. Contractor shall be required to provide the following documentation for all of his/her personnel working on VA property. A. Background investigation Worksheet (Attachment No.2 under SOP Acquisition) B. VA Form 0711 C. Security and Privacy Training certification D. OHSA certification E. TB tuberculin test certification 3. Contractor shall be required to have VA issued identification badges for all of his/her personnel Working on VA property. Anyone observed without VA identification will be asked to leave VA Property. No VA identification, no work, no exception 4. Contractor shall provide shop drawings for approval prior to start of work and as-build drawings after the construction is completed. 5. Contractor shall provide photos before and after the construction. 6. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA, Life and Fire Safety Codes, etc.). 7. There will be no smoking at this facility. 8. Contractor is responsible for obtaining his/her own vehicle parking. 9. Contractor is responsible for any damages caused by his/her personnel. All damages shall be repaired to previous condition or replaced with new at no additional cost to the government. 10. Contractor shall provide furniture, telecommunication and computer coordination. 11. Contractor shall provide his own storage for materials. The VA is not responsible for loss of tools, materials, equipment, etc. 12. Contractor shall provide construction signage at location of work. Signage shall comply with OSHA, Life Safety and NFPA requirements. 13. Contractor shall provide full time Quality Assurance/Quality Control inspector on construction site for daily monitoring of quality of work. A. Provide Daily Construction Quality Control Report containing preparatory inspection, initial inspection and follow-up inspection. B. Contractor shall obtain the services of a third party to provide independent tests required by plans and/or specifications 14. Contractor shall provide OSHA Certified Competent Person (CP) Safety Inspector to ensure that the specific safety requirements for construction operations are implemented during facility project. a. Provide daily monitoring and written report of compliance with safety regulations for construction site. Provide daily reports of safety inspections (include safety violations and corrective actions taken). b. Participates in the VHA facility multidisciplinary team established for the construction safety committee. 15. Contractor shall obtain the services of an independent Construction Safety Officer (CSO) to ensure safety requirements for construction operation is met. CSO shall be knowledgeable in the general inspection of typical work sites during construction and renovation performed by contract staff. The person shall act as the Safety Office in accordance VAAR 836.236-87 and VA Construction Safety Guidebook. This work includes: a. Collect deficiency information b. Disseminates actions and results c. Oversee of contract construction safety and review contractor safety program submittals. d. Determine if contractor is meeting VA standards and contractual requirements for safety and OSHA compliance. If/when the standards and contract requirements are not being met, in coordination with the VA COTR or CO, the CSO must take immediate action to prevent injury, exposure, noncompliance, or property damage. e. Requires Contractor's CP to implement and maintain an effective safety program and identifies and controls hazard that may cause injury or illness to VA patients, staff, visitors, and contractor employees. f. Conduct periodic inspections of VHA construction sites to ensure compliance with safety elements. At a minimum, weekly inspections are required with written documentation. g. CSO shall not take the place of the Contractor's CP or act on their behalf. Completion Time: 365 days (From NTP) The anticipated solicitation will be posted on July 31, 2013 Bid submissions shall be in hardcopies and not e-mailed or faxed. This project requires the bidders/contractors to secure bonding. All bidders shall be registered and verified in the www.SAM.gov, www.dnb.com/, www.vetbiz.gov and https://orca.bpn.gov/
