Description
Performance Work Statement (PWS) Keller Army Community Hospital Steris (Medium Steam Sterilizers) 1. REQUEST FOR INFORMATION (RFI): THIS IS NOT A SOLICITATION; no award will be made for this effort. This request for information is for informational purposes only; this is not a Request for Proposal (RFP). All interested parties are encouraged to respond to this RFI. The Government will not pay for information and materials received in response to this RFI and is in no way obligated by the information received. Companies who desire to participate in this market survey are encouraged to submit an information paper which supports their company's claim that it presently has the technology, qualifications, and capabilities to satisfy the deliverables described below. The information paper should discuss the company's ability to develop and or provide the requested annual maintenance service support for the Keller Army Community Hospital (KACH), at West Point, NY. 2. DESCRIPTION OF SERVICES: Require contractor to conduct a complete overhaul in order to return the Steris 3053 Sterilizer to safe and complete operation. Available upon request is a list of anticipated parts; however, it is not known if any additional parts may be required to complete the repairs. The two units affected are listed below. 1.ECN: D6087 Serial # 0129901-21 2.ECN: D6641Serial # 0124702-13 Anticipates man hours to repair these units are between 16 to 24 hours. Repairs will be completed within Building 900 located on Washington Road, West Point, NY. 3. HOURS OF PERFORMANCE: The contactor must coordinate work during normal duty hours, Monday through Friday 07:30 to 17:00. 4. CONTRACTOR RESPONSIBILITY: 4.1. The contractor shall provide a POC who can be reached in support of this repair. 4.2. If the contractor changes the POC, the government shall be notified as soon as reasonably possible. 4.3. Contractor personnel shall maintain a professional demeanor while performing services within a Government Facility. Each person shall wear a visitor's identification badge unless doing so would pose a personal safety risk in the commission of their duties. 4.4. The Contractor shall not employ current DoD employees, military or civilian, to provide services under this contract. 4.5. Contractor personnel operating motor vehicles on Federal property shall possess an operator's license of the category of vehicle being operated and automobile insurance as required by regulations/laws governing said Federal property. 4.6. Contractor personnel shall park in the appropriately designated parking areas as determined by the Chief of Warehouse Operations. 4.7. The Contractor shall comply with the Occupational Health and Safety Act (OHSA). Contractor personnel shall wear safety items required by OHSA during the performance of tasks requiring protective equipment/clothing. 5. SUMMARY: All work contracted under the terms of this agreement shall be performed during normal business hours Monday - Friday, 7:30 a.m. - 5:00 p.m. excluding holidays. Upon arrival at the facility, the contractor will report to the Security Manager's office located on the 1st floor behind the hospital information desk to sign in and receive a visitors I.D. badge and then report to the Medical Maintenance Section to record arrival and departure time from the hospital. The Medical Maintenance Section is located in the basement of the hospital next to Facilities Management Branch. Upon completion of services, the contractor will complete a joint inspection of the equipment with the Chief of the Equipment Management Branch. The contractor shall not perform services beyond the specified period or outside the work covered under this agreement without prior approval from the contracting officer. All questions regarding contract responsibilities shall be directed to the contracting officer for resolution. The contractor will not remove any of the contracted equipment from the premises without clearance from the Chief, Equipment Management Branch. Chief, Equipment Management Branch, (845) 938-3428, or his authorized representative is the designated representative of the contracting officer for the purpose of surveillance or performance and inspection of materials for work being performed under this contract. No inspector is authorized to change any provision of the specifications without written authorization of the contracting officer, nor shall the presence or absence of an inspector relieve the contractor from any requirements of the contract.
