- Page 2 SF18 to be completed by interested vendors NTQ-Specialty Clinics /sf18 page 2 DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Services for the Pine Ridge Service Unit In the Aberdeen Area Indian Health Service C-1 PURPOSE OF CONTRACT The mission of the Pine Ridge Indian Health Service Hospital is to provide the best possible health care services to the Native American population on the Pine Ridge Indian Reservation in South Dakota. To meet the mission, the Pine Ridge Service Unit requires contractor support to provide services in the Pine Ridge IHS Hospital. The contract effort shall be tailored to provide a wide range of specialty services. The purpose of the contract is for the recruitment, credentialing and placement of a Cardiologist(s), Endocrinologist(s), Nephrologist(s) and Transitionalist(s) (with each specialty providing their own midlevel(s), nursing staff and administrative staff) into the Pine Ridge Indian Health Service Hospital. Specialist services will be rotated based on the need of the patients. The specialty clinic will hold an average of (8) clinics per month with a minimum of (96) clinics per annum (accounting for months with holidays). The care provided by the Contractor shall be as comprehensive as the Indian Health Service supplied facilities, equipment and support services permit. The Contractor shall furnish the necessary personnel to provide specialty services to the Pine Ridge Service Unit. The contract resulting from this solicitation shall: 1. be a nonpersonal health care services contract under which the Contractor is an independent contractor; 2. allow the Government to evaluate the quality of professional and administrative services provided; but retains no control over the medical, professional aspects of the services rendered, (e.g., professional judgments, diagnosis for specific medical treatment); 3. require the Contractor to indemnify the Government for any liability producing act or omission by the contractor, its employees and agents occurring during contract performance; 4. require the Contractor to maintain medical liability insurance, in the coverage amount acceptable to the Contracting Officer, which is not less than the amount normally prevailing within the local community for the medical specialty concerned; and 5. require the Contractor to ensure its subcontracts for provisions of health care services, contain the requirements of the clause at FAR 52.237-7, including the maintenance of medical liability insurance. (See Section 1) C-2 GOVERNMENT FURNISHED PROPERTY, FACILITIES AND SERVICES The service unit shall provide office space including rooms for patient examination. The contract physician will be authorized to use all areas of the medical treatment facility available to civil service and commissioned personnel in similar positions. The same restrictions apply to the contractor physician as any other Indian Health Service personnel to use these areas for official-business activities only. The contract physician will be authorized to use all administrative and professional support available to Indian Health Service personnel. This will include but not be limited to copy machines, telefax machines, medical library, and telephone service. For the continuum of patient care, contract physicians will be authorized access to all patient records via the Electronic Health Records system. The same restrictions to use these items for official Indian Health Service business apply. The contract physician shall be provided with an orientation to the Indian Health Service Hospital. Service unit personnel shall provide the initial orientation and continuing orientation. The service unit shall provide all medical and nonmedical equipment and supplies used in the Pine Ridge Indian Health Service facility for the care and management of patients. The service unit shall provide all required blank Government forms applicable to the Indian Health Service. Service unit personnel will provide an appropriate orientation to the necessity of such forms. The service unit shall provide the authority for the contractor physician to purchase meals in the service unit dining room when available. The service unit shall provide an ongoing method of monitoring and evaluation of the quality and appropriateness of patient care and of identifying deficiencies in the quality of service performed before the level of performance becomes unacceptable. Generally, the service units will not provide patient transcription services. Legible handwritten documentation of medical records is acceptable in place of recorded dictation. C-3 CONTRACTOR FURNISHED EQUIPMENT, FACILITIES AND SERVICES The Indian Health Service does not anticipate a need for contractor furnished equipment. C-4 STATEMENT OF WORK The Contractor shall provide specialty services at the Pine Ridge Indian Health Service Hospital. The Contractor shall perform physician services compatible with the Pine Ridge Indian Health Service medical facilities operating capacity and equipment. The Contractor shall provide specialty services for Indian Health Service beneficiaries served by the Pine Ridge Service Unit. Contractor services shall cover the range of physician services typically provided in a rural civilian practice of similar size. Contract performance shall be according to the requirements contained in this Statement of Work. The standards of medical practice and duties of the Contractor through its specialty physicians shall be determined pursuant to the bylaws of the Pine Ridge IHS Hospital, the regulations of the hospital, applicable provisions of law, other rules and regulations of any and all governmental authorities relating to licensure and regulation of physicians and hospitals and the standards and recommendations of the Joint Commission on Accreditation of Healthcare Organizations. The Contractor shall be required to participate in a quality improvement program that is consistent with current Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirements for ongoing monitoring and evaluation of the quality and appropriateness of care. A Contractor shall not perform medical services until professional staff membership and the Indian Health Service Governing Body has granted clinical privileges in writing. As a prerequisite to performance under the resultant contract, the Contract physician performing the services must be privileged in accordance with Indian Health Service/Aberdeen Area credentialing policy. Therefore, compliance with the Indian Health Service privileging requirements is essential in the performance of the contract. Any failure to comply with IHS privileging requirements shall be considered to be nonperformance of the contract. The Governing body will not grant clinical privileges until it is determined that credentials information has been verified. Exceptions to the Indian Health Service credentialing review and clinical privileges process will not be considered. Any Contract physician who fails to maintain medical staff appointment and clinical privileges will not be permitted to perform services under the contract and shall be promptly replaced by the Contractor. The Contractor shall locate, credential, and have available sufficient staffing to provide specialty services on a regular schedule to the Pine Ridge Service Unit. Contract performance will not be required on Federal holidays. Prior to assumption of duty on any scheduled work shift, the Contractor may be briefed by the service unit medical director or his designated representative concerning any matter relevant to the Contract physician's assumption and discharge of responsibilities under the terms of this contract. The Contractor shall advise the medical director or his designated representative of any problem encountered, or which may be encountered, in connection with meeting the needs of patients treated by the Contractor. The Contractor shall report continuing problems of any nature to the Indian Health Service project officer. The Contractor shall generate and maintain proper medical record information on beneficiaries to whom treatment is provided in accordance with Indian Health Service regulations. The Contractor shall prepare all medical record documentation to meet or exceed the established standards of the IHS facility to include but not limited to: timeliness, legibility, accuracy, content, and signature. All medical record information generated in the performance of this contract will remain the property of and subject to the exclusive control of the Indian Health Service. Approval for off-site evaluation and/or testing will be obtained through the normal Contract Health Services protocols of the Pine Ridge Service Unit. Treatment of IHS patients shall be performed at the IHS facilities except in certain instances when patients are referred to private facilities for performance of special diagnostic procedures or definitive care not offered in the IHS facility. All referrals to private facilities or providers for specialized diagnostic studies or definitive care not offered at the Pine Ridge IHS shall follow Indian Health Service regulations and policies on patient referrals. The Contractor shall become familiar with and be guided by the Indian Health Service formulary. The Contractor shall follow Indian Health Service procedures when prescribing drug. The Contractor authorized to prescribe pharmaceuticals shall do so according to the availability of drugs listed the IHS formulary. IHS pharmacy services will provide instructions to the prescribing contract practitioner as to substitutions of generic drugs for prescribed drugs. The service unit pharmacy and therapeutics committee must approve drugs not on the formulary. The Contractor shall assess specialty services to assure its quality. The Contractor shall participate in a continual process of self-evaluation and strive for excellence in providing specialty services. The Contractor shall comply with all IHS facility infection control and safety procedures, practices, and standards. The Contractor shall meet all applicable Joint Commission on Accreditation of Healthcare Organizations standards during the performance of this contract. The Contractor shall respect and maintain the basic rights of patients, to include but not be limited to personal dignity. C-5 CARDIOLOGIST RESPONSIBILITIES The Contractor shall provide Cardiology services for patients of the Pine Ridge service unit. Cardiologist services shall include outpatient services as identified below. Specific outpatient Cardiology services may include, but not be limited to, the following: Life Threatening Emergency At the time of a clinical emergency any practitioner may render whatever care he believes to be indicated. Cardiovascular Disease Congenital/acquired heart disease Rheumatic heart disease Heart Failure, acute and/or chronic Cardiac arrhythmias Ischemic/Atherosclerotic heart disease Hypertension Metabolic and Endocrine Disease Fluid and electrolyte problems Diabetes mellitus Special Procedures (may be limited by the availability of services at the Pine Ridge IHS Hospital) Electrocardiogram interpretation Echocardiogram interpretation Chest radiograph interpretation CARDIOLOGIST QUALIFICATIONS The Contract Cardiologist shall be board certified, or board eligible without additional education or training, by the American Board of Cardiology. It is a condition of this contract that all required certifications in the Statement of Work remain current during the life of this contract. The Contract Cardiologist must hold a current, unrestricted state medical license from any of the 50 United States or the Commonwealth of Puerto Rico. The Contract Cardiologist must not have any exclusions or sanctions from the Medicare and Medicaid programs. The Contract Cardiologist shall meet the same privileging requirements as permanent IHS physicians before being allowed to provide medical services at the Pine Ridge IHS facility. The Contract Cardiologist shall not have any work or health restrictions that interfere with the performance of Cardiology services. The Contract Cardiologist shall speak, understand, read and write English fluently and communicate with sufficient clarity to be understood by Pine Ridge Indian Health Service Hospital personnel and patients. The Contract Cardiologist shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to patients from the Native American community. The Contract Cardiologist shall maintain standards of personal hygiene and grooming such that his/her image as a physician is compatible with the expectations of the Pine Ridge Indian Health Service Hospital clinical staff and the Indian community. C-5 ENDOCRINOLOGIST RESPONSIBILITIES The Contractor shall provide Endocrinology physician services for patients who require a category of Endocrinology care for complex and severe illness or problems with immediate or serious threat. Contractor services shall include outpatient and emergency services as identified below. Specific Endocrinologist services shall include, but not be limited to the following. Provide diagnostic consultation and management of metabolic and endocrine disease, including: A. HYPERTENSION 1. Essential hypertension 2. Malignant hypertension 3. Secondary hypertension B. METABOLIC AND ENDOCRINE DISEASE 1. Diabetes Mellitus 2. Diabetes Mellitus complicated by ketoacidosis or coma 3. Hypo- or hyperthyroidism, uncomplicated 4. Hypo- or hyperthyroidism, severe of complicated 5. Parathyroid 6. Adrenal 7. Pituitary 8. Gonad C. BONE DISEASES 1. Osteoporosis 2. Osteomalacia 3. Paget's Disease D. OTHER 1. Hyperlipidemia 2. The Contract Endocrinologist physician(s) must hold a current, unrestricted state medical license from any of the 50 United States or the Commonwealth of Puerto Rico. The Contract Endocrinologist physician(s) shall meet the same privileging requirements as permanent IHS physicians before being allowed to provide medical services at the Pine Ridge IHS facility. Current Basic Life Support (BLS) certification is required of the Contract Endocrinologist(s). The Contract Endocrinologist(s) shall not have any work or health restrictions which interfere with the performance of endocrinology services. The Contract Endocrinologist(s) shall speak, understand, read, and write English fluently and communicate with sufficient clarity to be understood by Pine Ridge Indian Health Service Hospital personnel and patients. The Contract Endocrinologist(s) shall process sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to patients from the Native American community. The Contract Endocrinologist(s) shall maintain standards of personal hygiene and grooming such that his/her image as a physician is compatible with the expectations of the Pine Ridge Indian Health Service Hospital clinical staff and the Indian community. C-5 NEPHROLOGIST RESPONSIBILITIES The Contractor shall provide nephrology physician services for patients who require a category of Nephrology care for complex and severe illness or problems with immediate or serious threat to vision. Contractor services shall include outpatient, and emergency services as identified below. Specific Nephrology services shall include, but not be limited to the following: Provide diagnostic consultation and management of renal disease, including: A. HYPERTENSION 1. Essential hypertension 2. Malignant hypertension B. RENAL DISEASE 1. Glomerulonephritis 2. Pyelonephritis 3. Nephrosis 4. Acute insufficiency 5. Chronic insufficiency 6. End stage renal disease 7. Dialysis patients 8. Nephropathy NEPHROLOGY QUALIFICATIONS The Contract Nephrologist(s) shall be board certified, or board eligible without additional education or training, by the American Board of Internal Medicine. It is a condition of this contract that all required certifications in this Statement of Work remain current during the life of this contract. C-5 MIDLEVEL RESPONSIBILITIES 1. Assess the health status of the individual by obtaining a complete history and conducting a physical examination 2. Request laboratory studies, EKG's, and radiographic procedures 3. Initiate requests for consultation and/or referral with appropriate physicians, clinics, or other health resources as indicated 4. Prescribe and dispense mediation and treatments as delineated by the pharmacy and therapeutics committee of the facility 5. Assess and/or initiate the immunization status of adults Care of Adults: 1. Assess and treat adults with minor acute episodic illness 2. Assess and manage patients with stable chronic or long term illness 3. Assess and treat common minor emergency problems such as bites, sprains, lacerations, etc 4. Counsel patients concerning common marital and/or family problems including crisis intervention techniques 5. Provide health teaching and counseling on an individual or group basis for patients with chronic illnesses or specific health problems 6. Assess and triage patients for immediate referral to the physician who have emergency life-threatening problems 7. Screen patients to determine need for medical attention 8. Review patient records to determine health status 9. Take a patient history 10. Perform a physical examination 11. Record pertinent patient data 12. Make decisions regarding data gathering and appropriate management and treatment of patients being seen for initial evaluation of a previously diagnosed and established condition. 13. Initiate requests for commonly performed laboratory studies 14. Identify normal and abnormal findings on history, physical examination and commonly performed laboratory studies 15. Perform EKG's 16. Cardio-pulmonary resuscitation MIDLEVEL QUALIFICATIONS The Contract Midlevel(s) shall be board certified, or board eligible without additional education or training, by the American Board of Internal Medicine. It is a condition of this contract that all required certifications in this Statement of Work remain current during the life of this contract. The Contract Midlevel(s) must hold a current, unrestricted state medical license from any of the 50 United States or the Commonwealth of Puerto Rico. Oversees the operations of the Pine Ridge Regional Medical Clinic 2. Helps ensures compliance between all involved parties (Regional Health Physicians, Oglala Sioux Tribe, Pine Ridge Indian Health Service) 3. Schedules and attends meetings with the Oglala Sioux Health Administration and other executives across the Pine Ridge Indian Reservation as needed. 4. Travels to various Oglala Sioux Tribe Districts to further develop and maintain relationships 5. Schedules and attends meetings with the Pine Ridge Hospital Executives to strategically plan on future endeavors 6. Ensure that the clinic operates with the Indian Health Service guidelines (in addition to Regional Health's) 7. Supervises all staff members in the patient care setting 8. Works with the physicians to ensure peer reviews are completed and provides the Pine Ridge Indian Health Service hospital Executive Team a copy of the resulting information Patient Navigation/Coordination: 1. Oversight of daily clinic operations providing patient navigation and coordination of patient/physician activities 2. Ensures optimal patient relationship and cultural awareness 3. Coordinates continued care between patients at Rapid City Regional Hospital and Pine Ridge Indian Health Service 4. Files patient records, updates files, schedules appointments and completes the appropriate paperwork for insurance billing, etc. 5. Ensures patients receive follow up calls and reminder letters about all upcoming appointments 6. Makes sure lab orders, radiology orders and pharmacy orders are entered. Communicates with patients to help ensure follow up testing is completed and then routes all results to requesting physicians 7. Enters and schedules all Contract Health referrals 8. Ensures providers are properly credentialed and maintain their credentials within the Pine Ridge Hospital 9. Point of contact for patients/physicians ensuring quality patient centered care is provided 10. Works with the physicians and the Pine Ridge Indian Health Service hospital pharmacy to ensure the drug formulary meets the needs of the patients 11. Will coordinate dissemination of information and provide information to Regional Health Physicians and to the Pine Ridge IHS hospital Clinical Director when necessary Additional Clinic Staff Registered Nurse: Provide and coordinate patient care, educate patients and the public about various health conditions and provide advice and emotional support to patients and their family members. 1. Record patients medical histories and symptoms 2. Record patients vital signs 3. Set up plans for patient care or contribute to existing plans 4. Observe patients and record observations 5. Consult with doctors and other healthcare professionals 6. Operate and monitor medical equipment 7. Teach patients and their families how to manage their illnesses or injuries 8. Point of contact for patients Licensed Practical Nurse: Provide and coordinate patient care, educate patients and the public about various health conditions and provide advice and emotional support to patients and their family members. 1. Point of contact for patients CHARACTER INVESTIGATION Individuals providing services on personal or non-personal services contract, the duties and responsibilities of which involve regular contact with, or control over, Indian children, are subject to a character investigation. IHS personnel offices will conduct these investigations following award of a personal or non-personal service contract unless in the judgment of the Contracting Officer, investigation may be waived based on prior investigations on file. Until the character investigation has been completed and the Contracting Officer notified, such individuals shall not have unsupervised contact with Indian children.
