- Enclosure 5: Corporate Experience Data Sheet Enclosure 4: Question and Answer Form Enclosure 3: Price Schedule Enclosure 2: Provisions and Clauses Enclosure 1: Performance Work Statement **** THIS ACQUISITION IS RESTRICTED TO LOCAL KOREAN COMPANIES LICENSE TO CONDUCT BUSINESS IN KOREA IN ACCORDANCE WITH FAR 25.4. **** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the Request for Quotation (RFQ); quotes are being requested and a written solicitation will not be issued. RFQ No. N40084-13-T-0068 and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-69. The NAICS code is 561210 This acquisition for services for the Commander Fleet Activities Chinhae (CFAC), and CFAC Family Housing, Korea to maintain, repair, and alter facilities, ground structures, personal property equipment and installed equipment and systems relevant to Vertical Transportation Equipment to ensure they are fully functional and in normal working condition, this includes developing, implementing and executing a Trouble Call, Maintenance Program, Inspection, Testing and Certification Program in accordance with the performance work statement, Enclosure 1. The following provisions apply to this acquisition: 1) FAR 52.212-1, Instructions to Quoters - Commercial, 2) FAR 52.212-4, Contract Terms and Conditions - Commercial Item, 3) FAR 52.212-5, Contract Term and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, 4) All other provisions and clauses included in Enclosure 2. The Price Quote Form has been provided in Enclosure 3 of the RFQ. Proposing contractors shall use the Q and A form provided in Enclosure 4 for presenting questions to NAVFAC Far East's point of contact, Ms. Wendie Narvarte. The end date for any question submittal shall be 03 September 2013, and questions submitted after that time may not be answered. Submitting questions is not required to qualify for award, but is recommended. Proposing contractors must download Enclosure 4 for certain provision terms that must be filled-in and submitted before the closing date for quotes. Proposing contractors shall provide completed copies of both FAR 52.212-3, Alt. I, and DFARS 252.212-7001, or they may reference a current Online Representations and Certification Application (ORCA) record. Proposing contractors are required to provide their Contractors' DUNS Number and Commercial and Government Entity (CAGE) code with their quote. The CAGE code entered shall be for that name and address provided by the Contractor. Quoters are required to be registered and current in the System for Award Management (SAM) database. This combined synopsis/solicitation is also available electronically on Federal Business Opportunities (FBO) website (https://fbo.gov/). In the event any amendments are posted via FBO prior to the due date, proposing contractors shall acknowledge receipt of all amendments at the time of submitting quotation packages. The Government anticipates award of a Firm Fixed Price contract. This RFQ is restricted to local Korean companies licensed to conduct business in Korea. The Government will award a contract resulting from this RFQ to the responsible quoter whose quotation conforming to the RFQ will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: A. TECHNICAL FACTORS Factor 1: Corporate Experience Factor 2: Past Performance B. PRICE Refer to Enclosure 2 for the RFQ requirements for each factor and the basis of award. SITE VISIT: An organized site visit has been scheduled for on 27 August 2013, 2:00 P.M. Arrangements to visit the site may be made by contacting during regular working hours at Yon.Chong.Song.KS@fe.navy.mil. a. Participants will meet at Commander Fleet Activities Chinhae, Main Gate at 13:45 to proceed to the site. All plan holders who wish to attend the site visit shall submit their list of participants no later than 26 August 2013, 10:00am to Ms. Yon-Chong Song via email at Yon.Chong.Song.KS@fe.navy.mil. b. Visitors will be required to present documentation evidencing personal identification and firm affiliation. A valid picture IDs for Korean Personnel includes a Driver's license, which indicates nationality or permanent address, a Passport or Resident Card with picture. c. During the site visit, it is each individual's responsibility to determine and wear appropriate Personal Protection Equipment (PPE) in accordance with the latest EM385-1-1 (Safety and Health Requirements Manual). If proper PPE is not used, the individuals may not be allowed to participate in the site visit(s) to the full extent. As a minimum, wearing safety shoes and a hard hat is mandatory all the time. All documents are due no later than Tuesday, 10 September 2013, by 10:00 A.M. Japan/Korea Standard Time (J/KST). Quotes and supporting documentation must be submitted by the deadline or they shall not be evaluated for award. All correspondence should be in English. Electronic copies of all quotation documents (technical and price) are hereby required. All electronic documents should be submitted in Adobe PDF format (*.pdf) unless an alternate format is approved by NAVFAC prior to the due date. Proposing companies may either submit their quotations by: a. Email. Quotation can be emailed to Ms. Wendie Narvarte at Wendie.Narvarte@fe.navy.mil. No email sent shall be larger than 4 MB in size. If the proposing company must send multiple emails, they must be labeled in the subject as Email x of n, where x is the unique number and n is the total number of emails. The time stamp of when the email is received shall be official, so please provide enough time when sending to allow for server delays. b. Postal or Delivery service. One original (1) and one (1) copy (exact duplicate) must be submitted to the addresses listed below. Please be advised that mail shall be recorded and dated on the received date, not the send date, so the recommendation for mailing is to ensure enough time for delivery to be made before the due date. All paper copies should be accompanied as well by an electronic version, and should be provided upon a CD or DVD disc (No Flash or Thumb Drives Please).
