- The Department of Veteran Affairs Facilities within the Veterans Integrated Service Network (VISN 7) intends to issue a Sole Source award to Abbott Vascular Inc. This requirement will be procured in accordance with FAR Part 6.302-1, only one responsible source and no other supplies or services will satisfy agency requirements. This is a requirement needed to provide Cardiac Catheterization Lab Equipment on a consignment basis. Performance must begin on or about 1 October 2013 through 30 September 2018. The contractor shall furnish all supplies as listed in Supplies/Services and Price/Cost on a Consignment Agreement basis to the Cardiac Catheterization Service, Charlie Norwood VA Medical Center (CNVAMC), One Freedom Way, Augusta, GA 30904. 1) The Contractor shall deliver FOB destination, the supplies on an as needed basis when requested by the Contracting Officer or designee. The designee will only be authorized to coordinate and request items direct from the Contractor. 2) The Government is obligated only to the extent of authorized purchases made against this agreement. 3) The unit price for each separate item shall remain the same, as identified in the Pricing Section, throughout the term of this Agreement. 4) All shipments made under this Agreement shall be accompanied by delivery tickets that contain at a minimum the following information: a. Name of Supplier/Contractor; b. Consignment Agreement Number; c. Itemized List of Supplies Furnished, including value of property at VA purchase price; d. Date of delivery/shipment. 5) The Consignment Items shall be subject to the terms and conditions specified on the warranty information for each individual item(s) and/or model number. At minimum, all items will be in proper conditions as required by applicable quality standards. 6) The Government will not assume liability for any of the items assigned to CNVAMC under this Consignment Agreement until such time as a requirement for the item exists and a purchase order is placed against the Agreement. Reimbursement will be made only if and when use of the item is made by CNVAMC. 7) As item(s) listed under Supplies/Services and Price(s)/Cost are used, a purchase order will be placed by the by the Purchasing Agent with the Contractor within 24 hours of its use at the price listed in Supplies/Services and Prices/Costs. This purchase order will authorize payment for the used item(s) and will also authorize delivery of the replacement item(s). All replacement items shall be delivered within forty-eight (48) hours from receipt of the purchase order. 8) During the consignment period, if any items(s) are returned to the Contractor, a signed receipt by the Contractor and the Contracting Officer's designee, listing the items(s) and the date of return will be provided to the Contracting Officer. At the end of the consignment period, all unused items will be returned to the Contractor at the Contractor's expense without reimbursement of other expense to the Government and a signed itemized and dated receipt will be provided to the Contracting Officer by the Contractor. The unused items will be returned to the Contractor within 10 working days after the end of the consignment period. This Notice of Intent is for information purposes only. All sources eligible please provide a capability statement in writing to the email in this notice. Responses must be supported with clear and convincing evidence to clearly articulate the ability to provide services outlined above. A request for documentation will not be considered as an affirmative response. If no responses are received, within 30 days after publication of this notice, to the effect that comparable services are available, and that it is more advantageous to the Government than obtaining these services through a Sole Source contract, then a Sole Source acquisition will be made to Alcon Surgical Inc.
