Description

All vendors should be aware that pages 62, 63 and 64 of this solicitation contain the past performance survey that the vendor needed to have filled out by their customer who the vendor has performed similar task for in the past. The form was not formatted by the system correctly. The contracting officer has attached a corrected revised version to this amendment which is a one page form. Both versions will be accepted with no deduction on the evaluation. If possible, please use the corrected version.
