- Solicitation for Group On-The-Job Accidents and Accidents To and From Work Insurance Services for Locally Engaged Staff The Embassy of the United States of America invites you to submit a proposal for On-The-Job Accidents and Accidents To and From Work Insurance Services for the Locally Engaged Staff employed at the Embassy of the United States of America located in Brussels, Belgium. This is a fixed price contract with economic price adjustments for the services specified and effective for the period stated in the Schedule. Your proposal must be submitted in an envelope marked "RFP SBE200-13-R-0006 enclosed" to the Embassy of the United States of America, Mr. Rafael A. Rodriguez, Regentlaan 27, 1000 Brussels, Belgium, on or before 16:00 hrs on October 11, 2013. No proposals will be accepted after this time. The Embassy intends to conduct a pre-proposal conference. All prospective offerors are invited to attend. See page 47 of the solicitation for instructions, date and time. In order for a proposal to be considered, you must also complete and submit the following: 1. SF-33 (Page 1 of RFP) (Offeror to complete blocks 12 thru 18, as appropriate). 2. Section B, Pricing Schedule. 3. Section B, Retention Amounts in B.3. 4. Section K, Representations and Certifications. 5. Additional Information a required in Section L. The Embassy intends to award a contract to the responsible company submitting an acceptable offer at the lowest price. We intend to award a contract based on initial proposals without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.
