PRESOLICITATION NOTICE - STRESS TEST SERVICES Place of Performance: Coatesville Veterans Affairs Medical Center 1400 Blackhorse Hill Road Coatesville, PA 19320 The Coatesville VA Medical Center is seeking qualified firms who may be interested in performing the attached medical services as identified in the Statement of Work excerpt listed below: The Contractor will provide the services of Stress Test to perform the services required within the Performance Work Statement at the Coatesville VA Medical Center (Coatesville VAMC), Coatesville, PA. DESCRIPTION/ SPECIFICATION/ STATEMENT OF WORK 1.Stress test, cardiovascular, using maximal or submaximal treadmill or bicycle exercise. 2.Continuous electrocardiograph monitoring and/ or pharmacological stress with physician supervision. 3.Interpretation and written report within seven calendar days of the test. 4.VA employee health staff will schedule the estimated 35 staff for the test. 5.VA point of contact is 610-384-7711 extension 4827, Department of Veterans Affairs, VA Medical Center, 1400 Blackhorse Hill Road, Coatesville, Pa 19320. SPECIAL CONTRACT REQUIREMENTS Under the authority of Public Law 104-262 and 38 USC 8153, the contractor agrees to provide Health Care Resources in accordance with the terms and conditions stated herein, to furnish to and at the Department of Veterans Affairs (VA) medical facilities, the services and prices specified in the Section entitled Schedule of Supplies/Services of this contract. 1. SERVICES: a. The services specified in the Sections entitled Schedule of Supplies/Services and Special Contract Requirements may be changed by written modification to this contract. The VA Contracting Officer will prepare the modification. b. Other necessary personnel for the operation of the services contracted for at the VA will be provided by the VA at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. c. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. d. The VA Chief of Staff or designee may provide professional direction to the contractor and/or the contractor's employee(s). e. There is no employer-employee relationship between the VA and the contractor or the contractor's employee(s). 2. TERM OF CONTRACT: This contract is effective for a period of one year from the date of award (with four one-year options, exercised at the government's discretion). The contract is subject to the availability of VA funds. The contractor shall perform no service after September 30th, each year, until the contracting officer authorizes such services in writing. 3. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall have required education and experience qualifications. Contractor's employees are required to be proficient in spoken and written English (38 USC Chapter 74). Please read the full Statement of Work contained in the solicitation which will be posted on or about August 29, 2013. Qualified companies who are interested in providing the services listed above shall send a statement of interest via e-mail to Mr. Wesley Kimmel at Wesley.kimmel@va.gov no later than September 13, 2013. The Statement of Interest must include the following information: 1. Company socioeconomic status (I.E. Small Business, Service Disabled Veteran Owned Small Business, Hub zone small business, etc.) 2. Completed Price/ Cost Schedule. 3. Completed evaluation factors as outlined in the Statement of Work. 3. Contact Information for the company to include name, phone, address, and e-mail address. 4. DUNS number of the company.
