- The Department of Veterans Affairs NETWORK Contracting Office 15 is seeking potential vendors for the purchase of a Ventricular Assist Device. Location: Department of Veterans Affairs St. Louis VAMC, 915 North Grand Blvd. St. Louis, MO 63106. If you can meet all of the requirements for this purchase, please respond to this sources sought by emailing Josie Manderscheid at josie.manderscheid@va.gov. Responses must be received no later than 4:00 P.M. Central Standard Time, on September 11, 2013. The following information is required: 1. Data Universal Numbering System (DUNS) & Commercial and Government Entity (CAGE) (This information can be obtained from your Central Contractor Registration). 2. Name of firm w/address, phone and email address of point of contact. 3. State if your company is a certified 8(a), Small Disadvantaged Business, Hub Zone Small Business, or Service-Disabled Veteran Owned Small Business. 4. Provide product brochures that demonstrate that your product can meet the requirements for this purchase. No telephone responses will be accepted. Statement of Requirements Saint Louis VAMC is requesting a Ventricular Assist Device (VAD). A VAD is a mechanical circulatory device that is used to partially or completely replace the function of a failing heart. VAD's are designed to assist either the right (RVAD) or left (LVAD) ventricle, or both at once (BIVAD). This would be used temporarily or for a short duration in urgent or emergent situations such as refractory cardiovascular collapse and failure to wean off cardiopulmonary bypass. A VAD differs from a standard "Heart- lung" machine in that the patient can be awake and ambulatory. Specifications: "Magnetically levitated pump impeller to minimize blood related complications (hemolysis) "Ability to provide left, right, or biventricular support. "FDA approved for extra corporeal membrane oxygenation "Ability to deliver high flows (at least 9.5 LPM) with a speed range of at least 0 -5,000 RPM "Need to include disposables (Blood pump, drainage cannula, return cannula), "Console, motors, cart, transporter, monitor, and all necessary accessories (such as brackets) Required Interfaces: "Compatibility with device (CentriMag/PediMag) utilized at local heart failure centers
