Description

The NF/SG Veterans Healthcare System, Malcolm Randall VA Medical Center, 1601 SW Archer Road, Gainesville, FL 32608 has a requirement for an Annual Contract for Cost per Test - Erythrocyte Sedimentation Rate (ESR) Analyzers. This is a combined synopsis/solicitation for commercial services and/or supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-68. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 500.00 employees. This requirement is a [Small Business] set-aside and only qualified offerors may submit bids. PERFORMANCE PERIOD: This contract will be a base year and two option periods, a total of 3 years. Each year subsequent following the base year will be exercise by mutual agreement. Each option year will be subject to the Availability of Funds, IAW FAR Part 52.232-18. The VHA NCO 08 - 573 - North FL/South Georgia HCS - Gainesville, FL requires the following items, Meet or Exceed, to the following: LI 001: Base year (12 months) for erythrocyte sedimentation rate analyzers for NF/SG VAMC Labs. CPRR/CPT shall be the same for each facility (GV-will run VIL specimens as well as GV, LC, TAL, JAK) based on full NF/SG testing volumes. Note: Base period begin from date of award through 12 months, 12840, EA; LI 002: Option Period one (12 months) for erythrocyte sedimentation rate analyzers for NF/SG VAMC Labs. CCRR/CPT shall be the same for each facility (GV-will run VIL specimens as well as GV, LC, TAL, JAK) based on full NF/SG testing volumes. Note: Begin from the ending date of base Period, 12840, EA; LI 003: Option Period two (12 months) for erythrocyte sedimentation rate analyzers for NF/SG VAMC Labs. CCRR/CPT shall be the same for each facility (GV-will run VIL specimens as well as GV, LC, TAL, JAK) based on full NF/SG testing volumes. This acquisition is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. The solicitation will be issued as a Small Business set-aside. The applicable NAICS code is 339112. The objective is to award a single Firm Fixed Price contract to a Contractor who can provide Non-personal services in accordance with the Statement of Work.
