Description

The Department Of Veterans Affairs, VISN 15 CMOP Contracting Office intends to release an Open Market Brand Name/Brand Specific commercial solicitation for the Charleston, SC CMOP. The solicitation is an EMREGENCY REQUEST for the medical supply DEXCOM G4 PLATINUM GLUCOSE SENOR. Due to the above information, the Delivery Lead Time for this request to be fully completed and to be delivered to the Charleston, SC CMOP, 3725 Rivers Ave, Suite 2 North Charleston, SC 29405-7038 NLT 10 days ARO. Description of required Med-Surg Procurement: "Description of request: DEXCOM G4 GLUCOSE SENSOR (Brand Name) "NDC: 008627004104 (NO SUBSTITUTES) "Item #: STS-GL-041 (NO SUBSTITUTES) "Quantity requested: 100 boxes (4 sensors per box = 28 day supply) "MUST CONTAIN THE FOLLOWING FEATURES: G4 PLATINUM SENOR; invasive (e.g. subcutaneous), disposable, for use with interstitial continuous glucose monitoring system up to 7 days "Supplier/Manufacturer: DEXCOM, INC. The NAICS code 339113 {Surgical Appliance and Supplies Manufacturing}, with a size standard of 500 employees. The solicitation number is VA255-15-Q-0382. All other contract details including full specifications will be included in the Request for Quotes, which will be released on or about January 30, 2015.
