Description: This is a Combined Synopsis - The North Texas Veterans Healthcare System located at the Dallas VA Medical Center has a requirement to provide two Community Based Outpatient Clinic s(CBOC) - one in Bridgeport, TX and the other in Granbury, TX. Offeror(s) will provide all facilities, staff and services as described in the attached Request for Proposal. The offeror(s) will provide both services and space for mental health and tele-mental health services. The CBOCS must be located in Bridgeport, TX and Granbury, TX. Offeror(s) are not required to submit proposals for both locations. The current Veteran patient enrollment at the Bridgeport, TX CBOC is 1550 and the current Veteran patient enrollment at the Granbury, TX CBOC is 1200. The resultant contracts will be for one base year and four option years. The NAICS code assigned to this procurement will be 621498 with a size standard of $20.5 million. The solicitation is full and open and multiple awards will be considered. Offers will be evaluated on technical ability, past performance, SDVOSB/VOSB Participation and price.
