Description

The amendment adds Evaluation Factors to Section E.6.1. of the RFQ to clarify Technically Acceptable specifications. NO FURTHER CHANGES. POC for this matter is Wayne.Geschwindt@va.gov. E.6.1 Evaluation Factors, Criteria, and Rating Methods 1.0. INTRODUCTION. 1.1. The Government will evaluate all proposals in accordance with the factors and criteria established in this Source Selection Plan (SSP) and in the Request for Proposal (RFP). The criteria provide the basis upon which each evaluation will be based, are established before receipt of the proposals, and are intended to ensure that the evaluation will be a structured process employing equitable measures. 1.1.1 The Government intends to evaluate proposals and make award without discussions. The Government reserves the right to conduct discussions if the Contracting Officer later determines discussions to be necessary. 2.0. EVALUATION FACTORS 2.1. Technical Ability 2.1.1. The purpose of the technical factor is to assess the offeror's proposed approach, as detailed in its proposal, to satisfy the Government's requirements in the SOW. Proposals must include the following: "provide residential placement and supportive services sufficient to place up to 25 Veterans under the Health Care for Homeless Veterans (HCHV) program, for the period identified in the SOW; "transportation to healthcare appointments and meetings; "contractor facility location within 200 miles of the Lebanon VAMC; "three (3) nutritionally adequate meals a day, 7 days a week; "laundry facilities for veterans to do their own laundry or to have laundry done; "structured group activities; "instruction in and assistance with health and personal hygiene; "provision of locked drawers for storage of medication; "supportive social services; "individual professional counseling, by a properly qualified individual who possesses a degree, license, or certification; assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships; "support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment; "assistance to gain and apply knowledge of the illness/recovery process in an environment supportive of recovery models. 2.1.2. This contract is designed to serve homeless Veterans in need of immediate placement and/or emergency housing in a safe environment. 2.1.3. Pass/FailPass/Fail Factor(s) _X__ will ___ will not be used 2.2. Past Performance. 2.2.1. The Government will consider each offeror's demonstrated, current, relevant record of performance in supplying services that meet the contract's requirement. Contracts completed within the last three (3) years are generally considered recent. 2.2.2. In conducting the performance risk evaluation, the Government may use data provided by the offeror and data obtained from other sources it considers current and accurate, including the Contractor Performance Assessment Reporting System (CPARS) 2.2.3.
