- OPTOMETRY SERVICES - CASPER, WY The Sheridan VA Healthcare System, Primary Care Division, 1898 Fort Road, Sheridan WY 82801 has a requirement for Optometry Services to be performed at the office of the contracted individual for a base period and 4 additional option years.The contractor shall provide Optometry Services in accordance with the specifications within this solicitation. PLACE OF PERFORMANCE: Contractor Facility, Casper, WY AUTHORITY: Health Care Resources in accordance with Title 38 United States Code (USC) 8153 to be furnished by the contractor on behalf of Sheridan VAMC. QUALIFICATIONS: The Contractor shall be duly licensed by the Department of Health of the State of Wyoming to provide optometry services, be a qualified provider under the Medicare Program, and be accredited by the TJC or equivalent accreditation. Personnel provided by the contractor shall provide the education and credentials of each clinical employee by name (C.V. and/or resume acceptable). CONTRACTOR RESPONSIBILITIES: The Contractor shall comply with all all requirements in the Performance Work Statement (PWS) included in the solicitation for servies. These include but are not limited to providing refractions, screenings, optical coherence tomography (OCT) evaluations and other eye evaluations if necessary. The contractor shall provide optical services to include measuring for eyeglasses, and performing eyeglass repairs, adjustments, and fittings. The contractor shall work with the Prosthetics Department of the Sheridan VA Healthcare System for the ordering of eyewear through the VA system. The Contractor keep appointment availability to < 14 days.The Contractor shall notify the COR of any clinic closures 45 days in advance. The Contractor Optometrist agrees to have 3 charts a month reviewed by peers to comply with the medical centers Ongoing Professional Practice Evaluations (OPPE). The charts will reflect diagnosis of age-related macular degeneration (ARMD), diabetic retinopathy and glaucoma. Contractor shall help the facility meet the goals specified in the Visual Impairment Prevention for Veteran Patients (USH Information letter 10-22010-007). Contractor shall facilitate appropriate and timely communication with patient's Primary Care Provider when Ophthalmological care of selected age-related macular degeneration, diabetic retinopathy, and glaucoma conditions is needed. Contractor shall ensure that optimal eye care is provided to all Veterans through a collaborative approach to diagnosis, treatment, and management of eye care by both Optometry and Ophthalmology. Contractor shall comply with semi-annual reviews done jointly by ophthalmology and optometry of up to six randomly selected patients diagnosed with ARMD, diabetic retinopathy, and glaucoma. The criteria for SVAHCS review will be based upon the Care Collaboration Agreements, which will be signed and agreed upon. Contractor shall be responsible for obtaining all necessary licenses. Contractor shall be in compliance with all federal, state, and local regulations applicable to the delivery of optometry services provided under the terms of this contract. Contractor shall hold certificate of insurance for general liability, malpractice insurance, and workmen's compensation. Contractor shall report all safety incidents immediately to the COR or the ACOS. Contractor's Personnel shall work with the Sheridan VAMC to ensure patient records and appointments are properly documented. Contractor shall contact the Sheridan Prosthetics Department to ensure eyewear is ordered timely and accurately.
